This just out today on the USAF web page:

http://www.brooks.af.mil/dis/4QTR04/LITERATUREREVIEW/dm1.htm

In a nutshell:

Sensitivity-very good (very few false negatives)
Specificity-not so good (fair number of false positives)

Take home point: The reviewers concluded that DIAGNOdent should not be
relied upon as a clinician’s primary diagnostic tool.  However,
clinicians could use the device after visual/tactile examinations to
provide additional information about the chances of disease on the
surface in question or to identify surfaces with demineralization to
signal the use of preventive services such as fluoride varnish or
sealants.

I like it for those cases which are right on the edge of the fence and I
need to decide if I want to tip it into the yard or out of it.  It is best
when used annually on all unrestored teeth.  Then, you have a trend to go
by, which is much better.  For those patients who you know are going to give
you a hard time about treating anything they cannot feel, it works very
well.  You let the patient hold the base unit and listen to the sound while
watching the numbers change.  If you explain what you are doing and what
tooth you are on, they quickly see the need to treat.  It is basically a
numerical explorer.  Instead of stick/no stick.  You have 0-100.  More data
to help in decision making.
