I had a lower left molar extracted, bone graft inserted and an implant
placed over the course of the past 4 months. A week after the implant
was placed, I went for my 6 month cleaning with my regular dentist. On
one of the x-rays that my dentist took, he noticed that there appeared
to be a small bit of left over root tip in the socket. Now my oral
surgeon has to remove the implant, find the loose piece, re-insert
bone graft material and then replace the implant.
My question is: Is this something that normally occurs or can occur?
Or is my oral surgeon just sloppy?

Unusual .......... not sloppy but perhaps not as diligent as he will
be in the future.

You deserve an explanation--not only as to if the root tip must come
out (as Joel and Steve M) say, but why the root tip wasn't noticed until
after the placement of the implant.
Sometimes root tips break off and the judgement is made that going
after it is more risky than just leaving it there. I have no problem
with that--and you are informed of it. If the surgeon decided it had to
be removed only after your regular dentist saw the root tip, I'd like to
know why the oral surgeon didn't discuss this possibility with you when
the implant was placed.
