Orthodontic retainer and bruxism mouth guard?

Hello,

I have worn an upper and lower orthodontic retainer at night since I
got my braces off over 12 years ago.  I am supposed to continue to
wear it for the rest of my life.

My bruxism has become bad enough that I'm chipping my front teeth and
exposing my gums.  My dentist said I should be fitted for a nighttime
mouth guard.  When I mentioned that I wear a retainer, he said — with
hesitation — that the guard would do the job of the upper (I believe)
retainer.  Is this true?  I really don't want to undo my orthodontic
work.

a bruxism guard and retainer are not the same thing but sometimes the splint
will serve adequately as a retainer.  I have had patients in which the splint
did not do an adequate job of retention and their teeth moved.

my suggestion to you is to talk with your orthodontist and see if you couldn't
have a wire cemented to the inside of your upper front teeth as a fixed
retainer.  you could then have a bruxism splint made with not fear.

That should work.

Last week I delivered a custom upper 3-3 NTI fabricated by a local (licensed
for NTI) lab.  It should work fine, the pt. destroyed a recently delivered
Essix with nocturnal clenching.  Sure would like to see NTI framework
extended distally for these cases.

It can be easily done in house with a vacuum former and splint material, which
you can get from essix. It's .40 thickness I think. You can use it as the
splint to maintain maxillary width & rotations, and add a discluding element
with orthodontic acrylic,

thanks for the kudos joel. never heard of these guys, we've always gotten that
stuff from raintree essix or great lakes.

I have done that as well. several times.  It took waaaaaaaaaay to long.
There is a local lab that does a nice job and is licensed by JB's company -
FWIW it was worth the additional expense.

Hi Jason,

You can check out night guards, which are separate from you orhodontic work.
I would recommend googling "TotalGard". They develop some of the best night
guards in the industry and are very affordable. I hope this information finds
you well!
