How to question sterilization procedures ?
Dear Doctors:
Wifey and I go to the same dentist. Last thursday she had her
teeth cleaned and ended up with 2 pea sized not spreading
inflammations around 1 tooth. A pus button on top. First time ever.
Very painful.
We went back and got a prescription for antibiotic and anti
inflammatory.
I am no expert but such an unusual response tells me there is
something wrong with the sterilization controls. But, I am just a
patient there.
Any advise? We like this doctor and would keep coming back. Thank you.

You might ask about his sterilization procedures. The dentist should
be willing to discuss your concerns.
The infection may have nothing to do with the sterility of the doctors
instruments. In the presence of periodontal disease, it is certainly
possible to innoculate some bacterial-laden debris or tartar into the
tissues, leading to infection. In any case, a discussion with the
dentist is in order.

Hi Dr Nunya,
Wifey and myself never experienced this before. Very unusual for us.
I have not asked and probably never will ask for evidence of
sterilization records showing proper sterilization procedures.
Mainly due to Murphy's law being easily applicable.

Personally, I have never seen such a nosocomial infection that you
describe. I have seen patients get a case of Recurrent Intraoral
Herpes or Aphthous Stomatitis either in the area of injection or along
the nerve tract. If the missus has severe pyorrhea, then perhaps it
got stirred up during the cleaning or perhaps it is only a
coincidence. Come to think of it, even in the old days, when some
dentists, carrying a mouth-mirror in their shirt pockets used the same
mirror between patients without sterilization, there were no cross-
infections.

Many years ago I bought a Lladro figure of a dentist. He had not only a
mirror in his pocket, but an extraction forcep as well.

If there is deep narrow pocket, it is possible for the area at the top
to heal sealing in the infection that was naturally draining.

Dear Doctors,
Everything is hanky dory now.
Thank you for showing your side of issues like this one.
Wifey got the infection under control and next day had
a cavity filled, on a different tooth.
However, in her own words: " I'd rather lose all my teeth
than going thru this again ".
Being consistent here, I think we have a fantastic dentist
( and dental technician ), but probably will use the
higienist services of independent dental higienist,
the ones that have their own business.

That brings up a question for me.......
Regarding independent practice of dental hygienists and denturists and their
acceptance by the general public. My State does not allow either, so it is
not discussed to often.
How do patients view these practitioners? As a dentist, I may be too close
to the forest to see anything but trees.

Independent hygienists don't seem to have been all that popular in CO
where it is legal. Over many years there have not been many practices
set up.

I was aware that not many practices were created, but that is probably due
to the high overhead to open any type of medical/dental facility and the
lower average hourly billing for a hygiene only practice.
I rather wonder how the general public sees the independent practice of and
RDH or a Denturist.

I'm not aware of any studies on that. Certainly there is a segment
that will seek treatment from illegal denturists. There is also a
segment who seeks treatment from form unlicensed dentists.

We have an illegal dentist or denturist splashed all over the news in our
area about once a year. I would not be surprised to discover that there are
dozens of them operating out of garages, cars, and spare bedrooms all over
Palm Beach County. It seems to be a cultural thing as much as it is an
economic thing. Simply put, immigrants from south America bring their
"dentists" from home. They have always used these folks, they can afford
their services, and many of them have no other options.
Then times here in the US get hard, they get even harder for immigrants
(both the legal and the illegal type). Things are especially bad for the
illegals because they have no unemployment benefits to fall back on. The
gaggle than hangs out near our local Home Depots hoping for an hour or two
of work is getting larger and more desperate looking.
