tightness in chest while taking Lasix

First of all let me say that I realize this is a question for the
doctor.  Unfortunately my husband has refused to contact him.

A week ago the doctor put my husband on Lasix.  He has fluid around
the lungs.  He has already had many tests trying to determine the
cause of the fluid and so far no luck.  Luckily they have ruled out
cancer.

The cardio thought it was possible that the fluid had a cardiological
origin.  My husband has afib.  He had quadruple bypass three years
ago, and according the the echocardiogram he had a small heart attack
some time during or right after the surgery.  This blew our minds
since nobody knew it until the echo.  It does not show on an EKG.  In
any case the cardio said the heart attack did not damamge the heart at
all.

So - he put him on Lasix.  This morning my husband tells me that he
woke up during the night and his chest felt like a "vice was
squeezing" it.  Of course he didn't wake me.  He said he drank about 8
oz of water and the feeling went away in about 10 minutes.  He told me
this has happened twice since starting the Lasix last week.

Does this sound like a reaction to the Lasix.  He does not drink
enough fluids, that's for sure, but he will now if I have to give it
to him rectally!   In addition to the Lasix he takes Lisinopril which
he's only been on for about a month.

We have an appointment with the cardio in three days.  Can it wait
until then?  What do you think?

Would suggest you contact your husband's doctor on his behalf as his
wife.

If I were your husband, I would not have called the doctor, either, when
I had the crushing chest pains. I would called 9-1-1. Crushing chest
pains in a patient who had open heart surgery is not something to wait
until the service gets a hold of the doctor on call. The ambulance will
be able to monitor you husband's heart beat (if he still has one), and
provide him with drugs or other treatments until the experts in the
emergency department can help him.

Folks typically do not relish the thought of a redo bypass surgery
after they have just had one.

It is unlikely you would be much different if you had "walked a few
miles in his mocassins."

Thus, it is understandable the reluctance to engage "emergency"
cardiac care especially after the bypass surgery experience.

Moreover, after the results of the COURAGE trial, we know that bypass
surgery typically does not help people live longer anyway so that
informing ones physician about recurrent angina rather than calling
9-1-1 is fine if ones physician is accessible.

This simply shows that the Holy Spirit is absolutely right to convict

Only Dr. Chung advises a bypas patient with "vice like chest pain" to
refrain from calling 911.

Emergency crews can quickly assess the patient and if there is no need
for an immediate trip to the ER will give the patient reassurance that
this is and event to contact his doctor about.

To imply that a layman can effectively diagnose himself is dangerous
and life threatening.

For Dr. Chung to make a broad statement about the effectiveness a
procedure he does not perform is inappropriate. He has no surgical
patients because he does not work at any hospital.
Practicing cardiologists who determine the need for bypasses do so
according to the patient's needs.  There is evidence that "one size
fits all" docs have over done bypasses.  Most Cardiologists treat
individuals.  They do not spend hours online spouting irrelevant and
dangerous advice based on limited knowledge and  experience.

We saw the cardiologist yesterday.  My husband was very pissed at me
when I made him tell the doctor about the tightness in his chest.
Tough.

He has been scheduled for a stress test in two weeks.  The cardio said
he's not very worried since the feeling only happens at rest and only
started since taking the Lasix and increasing his Lisinopril both of
which happened on the same day.  He guesses that either he was
dehydrated or that his blood pressure was too low.

I asked him if I needed to call 911 in the future.  He said if it
didn't go away within 30 minutes to call him.

If my husband should need bypass surgery again I'm pretty sure he
won't do it, and frankly I can't blame him.  He had a horrible
recovery from the first.  He was in the hospital for three weeks.  It
took him about a year to recover and actually he never did completely
recover.  He was 74 then and in great shape.  He's 77 now and feeling
old.  His memory was seriously effected by the surgery and never came
back completely.  Ditto for his energy.  I think another operation
like that would either kill him or end up making him a tired,
depressed old man.  It scares me to even think about it.

I'm praying that the stress test comes out OK.
