Hypertension , strange pattern .

I find my hypertension very unstable after I became T2 diabetic . My
diabetes is very much on the control with Metformin - twice daily .
Recently I was advised 1500 grams of calcium after bone density scan .
I find my BP is normal in the mornings and slowly builds up during
afternoon and after my dinner it reduces back to near normal levels .
What is this strange phenomenon due to ?

Any help and suggestions are welcome .

Kam,
I suggest that you copy and post your message in this newsgroup:
misc.health.diabetes
I also suggest that you read this book since it explains in detail
how the kidneys work.

Those people at the diabetes newsgroup might be able to tell you about
other books that could help you.

   I am on metformin for the last 3 yrs . I take one centrium silver
a day . I am a vegeterian but do eat eggs , cheese and dairy products .
I take long walks regularly . I hope this helps .

Kam,
Salt has major effect on blood pressure. Try becoming a total vegetarian
for a week--no eggs, cheese or dairy products. Eat lots of almonds (for
protein) or buy protein powder that has a very low sodium (salt) content
such as
Mixed Berry Rice Protein. After you do that, measure your blood pressure
during the last two days of the week to see if the experiment was a
success or failure.
You should also visit a health food store and buy a bottle of Mineral Capsules.
You should also ask these questions to your doctor.

Vegetarianism is a horrible choice for a diabtic.  Skip the rice crap
and eat more eggs to control bg.

 My sodium levels are lower than normal and
potassium levels are little higher than normal .

You haven't provided enough information here.  I would say, though that
your questions may elicit better informed ideas from
alt.support.diabetes than here.

What do you eat for meals each day?  How much of it is carbohydrate, how
much is fat, how much is protein?  MOST importantly, what are your post
prandial (one hour, especially) glucose numbers like when you test?  Do
your higher post meal numbers correlate with worse hypertension?  Do
eggs (you could have arachidonic acid sensitivity, leading to higher
bp)?  How much magnesium and potassium is in your diet?

Salt sensitivity is increased in diabetes which can be one possibility.

Salt-sensitive blood pressure--an intermediate phenotype predisposing to
diabetic nephropathy?

BACKGROUND: Family studies point to important genetic determinants of
diabetic nephropathy (DN). Blood pressure (BP) is higher in offspring of
patients with type 2 diabetes and DN, but the pathomechanisms involved have
not been elucidated. METHODS: We examined the salt sensitivity of BP after 5
days equilibration on a low (20 mmol/day) vs high salt diet (220 mmol/day)
in three matched groups of 15 subjects each: (i) control individuals; (ii)
offspring of type 2 diabetic parents without DN (DN-); and (iii) offspring
of type 2 diabetic parents with DN (DN+). Ambulatory BP and hormones
involved in sodium homeostasis [plasma renin activity (PRA), aldosterone and
atrial natriuretic peptide (ANP)] as well as the tetrahydrocortisol +
5-allotetrahydrocortisol/tetrahydrocortisone (THF + 5alphaTHF)/THE) ratio in
the urine as an index of 11beta-hydroxysteroid dehydrogenase type 2
(11betaHSD2) activity were analysed. RESULTS: In offspring of DN+ patients
on a high salt diet, systolic and diastolic BP was 137/82+/-10/8 mmHg vs
125/77+/-12/8 mmHg in offspring of DN- patients (P<0.01 for systolic BP).
The salt-induced difference in mean BP between high and low salt diet was
5.2+/-3.3 mmHg in offspring of DN+ patients vs 0.7+/-4.7 mmHg in offspring
of DN- patients (P<0.002). The proportion of 'salt-sensitive' individuals
was 67% in offspring of DN+ patients vs 20% in offspring of DN- patients
(P<0.05). In all groups, a high salt diet caused a comparable decrease of
PRA and p-aldosterone accompanied by an increase in ANP. The urinary (THF +
5alphaTHF)/THE ratio was 1.23+/-0.36 in salt-sensitive individuals and
0.99+/-0.33 (P<0.03) in salt-resistant subjects, consistent with increased
activity of 11betaHSD2. CONCLUSIONS: BP is more salt sensitive in offspring
of type 2 diabetic patients with diabetic nephropathy. The salt sensitivity
of BP may be an intermediate phenotype in individuals with a high risk of
future diabetic nephropathy.

How long ago did you discover this?

Are you using only one med for controlling HBP?  You may want to talk
to a doc about using 2 med for BP control.
