At what point should a 59 year old woman, 5', 145lb be considered a
candidate for near term aorta and valve repair?

When she has symptoms, compromised heart function, rapid progression of
aortic dilatation or there is evidence of sentinel bleeding.  Any or all
of these could justify a cardiologist's recommendations to proceed with
surgical repair possibly also with valve replacement.
