Request for the 990 Form for The Wellness Foundation, Inc

Attn Andrew B. Chung MD. Phd, incorporator and founding member of The
Wellness Foundation, Inc; a tax-exempt no profit public charitable
organization pursuant to Title 42, Chapter 17 of the Official Code of
Georgia . I formally request that you email to me a copy of the Form
990 and/or your tax reports for 2007. As a non profit 501(c)(3) are
required by law to disclose this information to the public and to do
so the same day of the request. You may post in on the internet in
lieu of emailing it to me.

This is to include all income, expenses, and salaries of officers as
required by law.

I have also sent an email request to the office of the Wellness
Foundation, Inc which is listed as Paradise Concepts in Mableton,
Georgia.

As per the contact page on the Wellness Foundation, Inc website I have
also submitted this request via email to "feedback@the
wellnessfoundation.com" since it was indicated that this was the
preferred form of contact.

Failure to respond to this request in a timely fashion and/or your
refusal to cooperate will result in the matter being brought to the
attention of the IRS and other proper authorities.

just got sentanced to two and a half years for tax evasion. Do you

Chung will never bow down to you and give you any information (even if
required by law).  In his mind such actions would belittle him.  He is not
above the law, but surely thinks he is above you.

Guess Chung will be adopting a 'wait & see' approach on that one.  He might
even stop posting for a few days if things get too hot (as he has done in
the past).

His mental illness and narcissistic, obsessive-compulsive personality
disorder will drive him to keep posting crap. But his intelligence is
deteriorating each year as evidenced by his inane and repetitive posts
of links that if read, show Chung to have been shown to be a liar and
a fool. I just want to be sure that the lies to the Georgia Medical
Board are corrected. He didn't disclose that he had lost medical staff
privileges in Florida, per info on the Georgia Medical Board site.

I also want to be sure that if he did receive 1 million dollars for
selling the worthless Emorycardiology.com domain name that it was
properly accounted for in either his own personal tax filing or else
in that of the Wellness Foundation, which is a public charity, tax
exempt, non profit. Let's keep Chung honest and accountable to public
agencies, even if he is crazy.

He will be sure to go on lying about being happily married, a
practicing cardiologist with collective clinical experience and
colleagues, having been beaten to death, having hundreds of thousands
of people successfully following his 2PD, an omer being 2 lbs, etc.
His medical license is under investigation now. If it is taken away
for reasons of dishonesty and mental illness, then things will get
even tougher for Chung. If he is found to be a tax fraud then maybe he
will learn a thing or two about telling the truth.

Chung's behavior on the groups would indicate that he is not too bothered
about keeping his license or not .

Most likely scenario is that cybersquatter, Chung, never sold
Emorycardiology.com for 1 million dollars, so there were was no money to
account for - meaning Chung could not evade taxes even if he wanted to.

Truth is simple ;-)

prayed thus with himself, 'God, I thank You that I am not like other
men--extortioners, unjust, adulterers, or even as this tax collector. I fast
twice a week; I give tithes of all that I possess.' And the tax collector,
standing afar off, would not so much as raise his eyes to heaven, but beat
his breast, saying, 'God, be merciful to me a sinner!' I tell you, this man
went down to his house justified rather than the other; for everyone who
exalts himself will be humbled, and he who humbles himself will be exalted

Not to support Chung, but your request here and any response from Chung here
probably has no legal meaning.

You need to wait until they respond to your email to the email address given
by the Foundation.

If you do not hear back and are serious, then to be sure and have better
prove, probably best to send a certiified letter with signature required so
there is no question of the email not being received or if you fax it, it not
being received.  The signature card showing it was received or it being
returned, is much better.  In your request put down the certified number in
your letter requesting the information.

Obviouslu this is something Chung is going to turn right over.

One more thing to try before the certified mail, if the Federal Agency has an
email address, and do not hear back from your first email, send a second
request with a cc to that government agency

Thank you for your suggestions, but you are making this way to
complicated. It's simple. Suspected tax fraud can be reported
anonymously. The IRS will sort it out. No certified letters required.
Chung's Internet lies and boasts are about to come back to bite him on
the a.s in real life. Truth is simple.Chung is a liar and a fraud.

Seems there are two issues. First is tax fraud for not reporting the sale of
the domain (assuming it sold for anything), which the IRS will likely
investigate without the need for you to comply with any formalities. Second,
the failure to comply with your request for Wellness Foundation docs. I'd
think you need to be able to show you've jumped through all the required
hoops and that the officers had proper notice for the IRS to take any
action.

Hi Zen. My goal with the IRS would be 1) to be sure that the Wellness
Foundation has reported form 990 for the tax year of 2007 as a tax
exempt nonprofit and 2) to alert the IRS to Chung's claims of
receiving 1 million dollars last September for selling the Internet
domain name of emorycardiology.com. and 3) to alert the IRS to Chung's
claims that he already had a 1 million dollar guarantee and it is now
doubled after the sale of the domain name.  It is either reportable on
the Wellness Foundation's returns or else on Chung's personal returns.
If there is no actual sale for 1 million dollars then Chung is lying.
It is up to the IRS to sort it out.

You are correct that they are two separate issues. The concern that my
request for the public disclosure of the 990 was ignored is yet a
third concern. The IRS accepts anonymous reports of suspected tax
fraud. The fraud would be 1) the Wellness Foundation not filing this
year or 2) actually having received 1 million but not reported it.
Additionally, if the original unprecedented 1 million dollar guarantee
funds are held somewhere, they certainly are earning interest and that
should also be reported as income. The IRS can make sure that this is
taken care of.

If there are no actual funds to back this unprecedented 2 million
dollar guarantee, then Chung is only guilty of lies and deception, but
not tax fraud.
