I have had recurring prostatitis over the years brought on by excess alcohol
on weekends and vacations and a lot of bike riding.
I am 58. My PSA at the time of the last episode of one drop of gross
hematuria was 3.9 and free PSA was 19%.
DRE is negative for PCA but reveals warm, boggy prostate, cystoscope
negative. Family history negative for PCA. Renal and pelvic ultrasounds
negative. Prostate enlarged 25cc based on pelvic ultrasound. Some post void
urine retention due to BPH.
I stopped all alcohol and bike riding. Took 10 days of Levaquin.
Three weeks later and still no booze, no biking and no spicy foods, PSA is
2.9 (lower than a year ago when it was 3.1) and free PSA remains 19%. I have
NO symptoms anymore.
Doctor says he wants a sonogram and possible biopsy just to be sure based on
free PSA.
Is this necessary? We have no baseline on my free PSA. It might have been
19% when I was 21! We do know my PSA is headed lower nit higher and there is
NO PSA velocity to worry about here, per the MD.
I asked MD if TRUS could wait till after the first of the year and he said,
by all means. I suggested monthly PSA and free PSA tests till then and he
was fine with that.
I will do what needs to be done but do not want to be subjected to
unnecessary tests either. And, of course, I am terrified.
I am getting a second opinion next week from another uro. Thoughts anyone?

In your shoes, I'd be very happy to forego a biopsy at this stage -
especially since you say your PSAV is zero or negative. If you had
cancer, it would be slow enough to give you time to think anyway. A
monthly PSA test might take a heavy toll on your nerves - 3-monthly is
more common and will give you time to forget about your prostate in
the interim. My POV only.

Biopsies are nothing. Tell them to give you happy juice and you won't even
know it's happening.
