Oh, the elbow's bad enough, especially given your other problems. But the sad part's the last sentence. Few of us can truly feel your anguish, but we hear you nonetheless. Keep up the fight, big guy. It still sounds like your heart is still strong even though your carcass is slip-slidin', and, after all, your heart is what we hear. Many of us would like to have had the inner strength 30 years ago that you exude even now. Radiotherapy can palliate bone pain and mend areas of bone before they break. Get assertive but keep it simple. Go to a radiation oncologst and tell him/her you want to followed (and treated, where possible) on an ongoing basis. Otherwise, it's breaks for you hereonin.
