The free PSA can be detected when total PSA is less than 2.5, but
studies showing fPSA to be a useful indicator (e.g. if % fPSA < 15%,
then that's an indicator that PCa is present) have not included men
with tPSA below about 2.5

>From my readings of this and other PCa NGs, it seems a fair number of
men get a fPSA test during the diagnosis process. The PCA3 test has
been shown to be a sensitive test for PCa, but the problem with it is
that it requires the doc to due a "vigorous" prostate massage in order
to express the protein being detected. Unless you use a doc who really
knows how to give the "PCA3 massage", results can be equivocal.
