You raise an interesting point. Many men who have been on ADT for 24 months
take a long time to or sometimes never regain T production. It has probably
been mentioned here before in posts that I've glossed over that having T
checked after 24 months may save you lots of Lupron cash.
As to gynecomastia, it is relatively easy to defeat if hit immediately with
radiation (4 cycles) or with some meds. However, "once a significant degree
of gynecomastia has occurred, it does not respond well to treatment with
radiation therapy nor does it significantly disappear when" ADT is
topped. -- Strum, p. 150

I've had big boobs my entire life, getting bigger with age. I am self
