Painful urination, muscle cramp before urination

A friend has undergone 9 months hormone treatment (Zoladex) and is in his
4th week (of 7.5 ) of conformal radiation treatment and is experiencing
excruciating pain prior to and during urination - urination can take him 4
minutes or more.    With medical advice he recently started using a
self-applied catheter to assist in emptying his bladder - this procedure is
extremely painful both for insertion and withdrawal.    Can anybody offer
any advice to assist please?

It may be too late, and perhaps he is already doing it, but I drank three or
four liters a day, starting a couple of weeks before radiation, to
counteract the effect on my urethra.  He probably doesn't want more fluids,
but it's the best way that I know of.

Drink plenty of water and sit in hot water to urinate. I use to sit 5 to 6
times in 24 hr period - day & night. I am surprised that his Radiation
Oncologist has not apprised him of having a full bladder during
radiation....
