Brachy and Enlarged Prostate

I have an enlarged prostate and corresponding urination problems (a couple of times
at night, weak stream etc).
I know that if I chose LDR Brachy, I should expect more severe urination problems for
the next 6-8 weeks after implantation (maybe also a blockage and a need for temporary
cateter). But I did not find any info about long term effecfs: say, after 6-8-12
months will the prostate shrink enough so that the pre-implant  urination problems
resolve ? or at best, it will be as before the implant ? or even worst ?
Any experience with this expect of LDR Brachy ?

These are good questions to ask your urologist and your radiation oncologist.

I had HDR plus EBRT and had urination problems for about 5 months that
were pretty well controlled with Flowmax.  But my prostate wasn't too enlarged
before treatment and I had no urinary difficulties beforehand.

I did have prostatitis before treatment and still had it afterwards.  It wasn't
severe and hasn't been a problem for me, but I admit to hoping that
the radiation would cure that too - but it didn't.

Dr. Walsh says in his book that if you've got other prostate problems,
surgery might be better than radiation because, by getting rid of the
prostate, it solves all of them.

I was agreeing with Dr. Walsh's suggestion, when I was told I would be
wearing the Foley catheter for 3-4 wks post discharge after HDRB.

However, my Radiation Oncologist assured me that my Tumour was far too
aggressive (4xG8 plus 2xG7), plus my age (70), other ailments would
make the 18 mths ADT, 23 fractions of EBRT plus 3 doses of HDRB the
better treatment regime, given the risk of a temporary Urinary
Blockage for a few weeks post HDRB.
(You possibly know I got all this on the Clinical Trial.)

I'm peeing OK now with the help of Flomax.
All I want to do now is get a clearance from the evil "Catheter Lady",
hopefully today 08/08/05 and get back to work to recover a few $$$.

Then if necessary, have yet another TURP later on.

Had the same problem with  enlarged prostate (72cc) from BPH which had
been TURPed 18 yrs ago, but still caused a blockage after my HDRB.

Big day for me today (Monday 01 Aug..)
Finally got that damn Foley catheter out after 3 wks and passed the
voiding trials at the local community health centre. "woohoo"!

Had to return for another set of "normalising" trials on Thursday to
check the bladder sphincter has resumed it's old habits. This was OK,
but the urinary retention was still too high, so I have to return
Monday 08 Aug to have yet another bladder scan.

I explained to the nurse that my retention was always high prior to
the HDRB, so she's going to give me some "self-catherisation" kits in
case I get into trouble. (Damn glad that Foley's not going back in).

Still on Flomax until further notice, plus another TURP may be on the
cards after follow-up with the Uro who placed my HDRB template and
catheter needles.

I was 62 when diagnosed, and had a prostate volume of about 60 cc. The Uro
had me on Lupron for about 6 months prior to seeding to shrink the gland,
the volume at seeding time was about 23 cc. I experienced urinary retention
and urgency for a while following the implants with the worst about 6 weeks
post seeding and then improving. I was taking Cardura for blood pressure, so
no Flomax ( both are alpha blockers), just an increase in the Cardura for a
while which helped.
At 14 months post seeding I normally get up once during the night, a lot
better than the 3-4 times that drove me to see a Urologist in the first
place.

I'm not a doc and I have no idea how big is an enlarged prostate. Seeding
usually will cause the prostate to swell. (It really pitches a fit being
invaded with seeds.) But after the swelling goes away so does the prostate.
The prostate gets fried to death with the seeds. After about a year or two
there is nothing left but seeds and a little scar tissue.

Bev,
I don't know for sure what gets fried, broiled. sautéed or baked, but !!!
now that the Lupron has worn off, I'm glad that it's only medium or medium
rare, not well done.

BTW we got halfway to your area [God's country is on
side of the tunnel/bridge ;-)] over the past weekend

Am I to assume you were on the Eastern Shore? Yes, it is lovely there. Wish
I could afford to live in some old Victorian in one of the little coastal
towns.

Yes we were, one son and his family live near Salisbury.
My thought was the north side of the tunnel/bridge, I guess that's why
there's a need for proof reading.
