Had my first PSA post March 24th RP. Nurse said that lab would send me
the results and my doctor set me up for another PSA in 3 months. Should
my first PSA post RP be indicative of what I should expect if everything
went the way I am hoping it has or should I not sweat the first and wait
until the second PSA and see if there was a rise in PSA between the two?
Also my doctor stated that he saved the nerve bundles but doesn't know
what effect the surgery may have had as far as erections goes. Since I
haven't been sexually active since the death of my girlfriend from lung
cancer in 2003 he still wants me to take a 50mg Viagra tablet everyday and
gave me 3 sample packs (18 tablets) to take. I don't expect to be seeking
female companionship during the course of these 18 tablets so I am
wondering if the Viagra was given just to see if erections are possible
through visible stimulation or what?
Lastly, it has been 26 days since my surgery but I am still passing blood
every time I urinate. Doctor didn't seem to be surprised at this but he
did take a sample to run a culture for infection. My question here would
be, is it common for one to still be passing blood a month after surgery?
Also he asked if it still stings after I urinate which I replied yes.
Again, one month after surgery is this a common occurrence?

After RP, the first PSA reading after 90 days should be undetectable.
If the doctor told you you had clear margins, you should already not be
sweating this.

As already mentioned by Jim - 30 days after RP
the PSA should drop below 1 and stay there for years to come
Viagra pills are not vitamins and will have no effect unless you are
in bed with a female preferably naked
Urine should be clean of blood after 14 days from RP,
unless you exercise a lot - in which case relex a bit and let it all
heal

Hi Jack.
I'm having my first PSA post surgery on Friday.
This will be almost 6 weeks after the March 21st. surgey date.
With clear margins and no lymph node problems I'm hoping for an
undetectable also.
I still pass small brown clots occasionally which is older blood but on
3 occasions...I passed a red clot and a gush of red urine.
It stopped right away as my next urination was OK.
I don't know if it was from carrying groceries or bending funny but some
of us aren't healed yet.
About Viagra...I will be asking for something to get the blood flowing
back to the penis as was mentioned earlier.
My doc likes Cialis but I don't see him for 2 more weeks.
I'll have to either call him...or "get busy" on my own.
Physical stimulation seems to be the way to go.

Hoping for the best on your first PSA. I too, in addition to passing
blood, pass both dark and bright red clots at times. Since I have no one
else to help with things around the house the passing of blood this late
may indeed be just doing everyday things that others with help may be able
to avoid (cleaning, cooking etc.)
Just got a call back on the path report and the margins. Lymph nodes and
seminal vessels were negative as well as all incisional margins except for
one area that showed, "atypical cells that could be an extension of the
tumor, difficult to determine". I think this means that the pathologist
doesn't know for sure one way or the other.
I guess the PSA tests will determine what the future may hold but it would
have been nice to get a path report that was clearer than the one I got.

Beg to differ here. There are some indications that taking Viagra regularly
after RP may hasten erectile function. I believe a study was done on this.
This is probably why your uro wants you to take the Viagra.

PSa has an in vivo half-life of roughly 3-5 days depending upon the
individual. Most people say it takes about 10 half-lives to remove the
PSA that was a) in your bloodstream prior to surgery and b) released
into your bloodstream by surgery. So a PSA measurement at only 26 days
post surgery may yield a value higher than what will be measured at
your next 3-month blood draw, but it should definately be lower than
your pre-surgical PSA.

Studies have shown that V promotes blood flow into the nerves that may
have been damaged during surgery. Blood flow will promote their
recovery, thereby making erections down the road more likely.

Sounds like a long time to me.

Sorry about your GF Jack. If your margins were clear, your PSA will be
undetectable. You learn to sweat out each result as the months and years
roll by, and this all hopefully becomes a bad dream.
Some are doing this early Viagra thing, but I haven't seen any postive
conclusions drawn yet from anyone. Most of us try it after a couple of
months post RP and even then the results are spotty. The visual stimulation
is almost nill from everyone I've spoken too. I'm almost 3 years post RP and
get erections only with stimulation. You'll need to try and get intimate
with your little pal as soon as you can to get him ready and willing to go
again.

Thanks for all the replies. They have been helpful. My doctor has been
vague about the margins only saying that the cancer was more 'extensive'
according to the path report but he 'believes' that he got it all. He did
note that my Gleason remained the same 3+3. My pre-op PSA was a 3.3 and
my
staging was T2c. Today I called his secretary and left a message for her
to
find out from the path report and call and tell me if the margins were
negative or positive (still waiting for the return call) .
I believe the blood that I am still passing may be from the damn catheter
which had to be removed a day early (13 days ago) because of problems
with
it functioning properly. My urine was almost completely clear before the
catheter's balloon stopped it from working properly and after going almost
24 hours passing almost nothing but small amounts of blood it was removed.
The stinging had gotten better also until this happened.
As far as the Viagra, I guess I will take them as instructed unless I
experience some serious side effect.

Jack,
Rather than have her tell you over the phone, get a copy of the report and
read it yourself. Or, get it and type to us the exact wording. Some of us
are pretty good at interpreting medicalese. Especially our resident MD.

Jack,
I think you need to shift down on your brain. You're worrying way too much
if you're worrying about this PSA and the next PSA and the difference
between the two.
Worrying about this PSA result is a fact of life. We all went through it.
Most of us still go through it. After RRP, you should expect a <0.1 PSA.
Especially if you're a 3.3 PSA going in and your doc salvages both nerve
bundles. If this one is not <0.1, I'd be surprised. If the next one is
not, I'd be astounded.
However, you should not be waiting this long for your 3/24 results. Call
your doc.
And call your doc about the peeing problem. Probably a minor resolution to
it.

My doc said the PSA at 1month should be <0.1. It was.
I started the injections day 13 post op. I used them 3-4 times a week
to have sex. They were great! I think they helped get my erection back.
I started using the pills at about 3 months on nights I was not using
shots. At 14 months I have about 75 % of my erectile function back, and
improvement continues. My doc said use it or loose it. I agree.
I had intemitent bleeding for 8 weeks. At 6 weeks I still felt like I
had sand in my urine, so I thought I must have an infection. A culture
was negative. The doc said he would give antibiotics anyway if I
wanted, but suggested waiting. I cleared up spontaneously at about 2
months, and my urinary function was excellent.
