Surgery after radiation

Received wisdom states that surgery after failed radiation is
not practical.  Curtis Palmer once offered a wonderful explanation
of why involving two slices of bread, a lump of cheese, and
a microwave oven.

However I now see that there is a clinical trial of prostatectomy
after radiation underway at many sites all over the country.  See:

Anyone in this study?  Anyone have surgery after radiation?

I'm not in any study, plus I haven't finished my radiation course as
yet.
However, when I complained to my Rad Onc the other day of throttled
flow, he has referred me to see a Urologist, prior to the HDR Brachy.
He didn't give me a reason, but I'm hazarding a guess about the
possibility of another TURP, as I have read of this in a p2p post.
A Uro has not been on the menu in my current treatment regime up until
now!
So it begs the question, and then again, it may not.



He also gave me Flomax to ease the present throttling. It's worked.

I'm guessing that if the Flowmax works, a TURP won't be needed.
I wouldn't think the Flowmax would do much if there was
scar tissue blocking the flow.  And if it's just inflamed tissue,
that should recover sometime after the end of radiation, though
it could take some months.

In my case, I had a slightly enlarged prostate before starting any
treatment. About 1/2 way through my  IMRT's I began "slowing up" due to
swelling of the prostate. My URO went in and "stretched" things a little,
which helped for a few days. He said that he would not consider any surgery
(TURP,etc) until long after I had completed Radiation. I was originally set
up for 25 IMRT and then Theraseeds. But he was afraid that since the Rad was
causing swelling that the seeds would definitely cause more cutting off the
flow completely and at that point all he could do was to give me steroid
shots(requiring hospita;ization) to try to bring the swelling down as
surgery can't be performed after the seeds are in place. So we decided to
forgo the seeds and do 42 IMRT instead, which the RAD ONC said would be
equivilant to 25 IMRT plus seeds. I have been taking 2 Flowmax a day for
about a year and have a good flow. I finished RAD in June 04. Will continue
the Flowmax for a while yet and then have a TURP or will just stay on
Flowmax since I am still having some side effects from the Rad.
our URO will probably do a Cystoscopy which involves going in and taking a
look see and also a little streching to see if that helps. In can be done in
his office or as Hosp outpatient(which is the route I took. I doubt if he
will attempt any surgery while you are getting RAD or seeds.

No seeds due to Prostate problems
8/30/04 - 1 yr Viadur Implant instead of 4mo Lupron
1/14/05 - Removed implant - trying intermittant HT for a while.

I'm guessing from Alan's and Olfart's subsequent replies that the Uro
might just want to poke around a bit and "stretch things".
My prostate was 72cc on entry to the program Nov 1 2004, and a
pre-treatment External Ultrasound was showing a post void residual of
158cc. The ADT has had me up at night in 1-2 hrly intervals and after
only 3 fractions of XRT, I was just dribbling.
The Flomax now has me back to pre-treatment intervals, which were by
no means like a draft-horse!
