steps to urinary continence

In three weeks my husband, who underwent RRP yesterday, will have his
catheter removed and begin his quest for urinary continence.  This could
be difficult for him to achieve as his bladder muscle tone is poor,
capacity is limited and urinary urgency is already present due to many
years of BPH.

During my years of teaching I have learned to make tasks easier to
accomplish by breaking them down into specific goals to be met through a
series of small steps.  I would like to devise a program that will
enable him to meet small measurable criteria and prevent him from
becoming discouraged because of unrealistic expectations.

I'm hoping to use Dr. Walsh's phases as a framework:

Phase 1:  Remain dry when lying down
.
Phase 2:  Remain dry when walking around
.
Phase 3: you are dry when you stand up after sitting

I hope to incorporate timed voids, as often as necessary,  to maintain
low bladder volume and pressure.  Exercises for strengthening and
controlling the remaining sphincter would also be included in the
program.

I would appreciate any suggestions from group members who have "been
there and done that" in developing these small, hopefully achieveable

Greetings Leah:
   Your husband is lucky to have you.  I understand the stepped goal
approach and the need for realistic expectations.  My catheter came out last
Thursday and I was really disappointed to find myself totally incontinent.
I am 56, and I did have some problems with urgency, etc., in the last two
years.
   So, I cannot really help you much because I am not even one week into my
objective to get Depends off the shopping list, but if you are not already
familiar, take a look at http://www.phoenix5.org/ for information on many
facets of this disease, including dealing with incontinence.  Note the Kegel
exercises.  Everyone tells me that I am in for a 3 month experience, with
slow recovery, and that the Kegel exercises will speed things up.  Walsh
says it can take one year.  There was a post from someone this week who is
in 1.5 years and still has leakage, so we are all different.  Who knows,
maybe your husband will be one of the lucky ones and start off with only
minor leakage.

 Your approach may be worth publishing, if it does in fact produce good
results.  No one has made such an exhaustive study and/or approach that I
have ever seen. It took me over 6 months to be dry, but I've had better than
average erectile function from the start than most, so I never complained.
My success is from simply stopping and starting the stream every chance I
get. I think it's as good as doing the kegels.
