Long history, now chemotherapy.

72 y/0. 1988 TURP for BPH showed 3+3 PCa. EBRT. Routine PSA just new.
Recurrence February 1999, PSA 5.9.
Casodex, Zoladex (changed to Lupron). Casodex dropped, DES (up to 3mg)
added, Proscar just because.
November 2003 PSA 8.3
January 2004 28.7
February - minimal ascites noted
April - biopsy of solitary nodule on shaft of penis - positive.
May, June - radiation to penile nodule, inguinal chain
PSA May 43.5, June 58.4
July Distended abdomen. Paracentesis - prostate cancer cells, PSA 91

Taxotere July 21, July 28. Brief hospitalization for FUO, presumptively
peritonitis, though not proven. Five days iv antibiotics as out-patient.
Next Taxotere will be 8/18

But - PSA 63.7
Onward, 16.5 years later

Larry, sounds like you've been through the mill with all of this crap.
I have a question about the ascites in Feb. How was that diagnosed?
I asked Curt's Onc. dr about his distended abdomen last month (in that magic
8 min of time he gave us) and he said he didn't notice anything. but he said
"we'll see what the PSA is (his standard answer to everything)" it was down
so that was good but Curt is very distended. his waist size is up from 34 to
approaching 38! He says he always has a "full" feeling and feels "bloated"
especially after eating even a small amount
I know someone will say to get a new doctor but I haven't been able to get
Curtis to switch or get a second opinion.

Lorelei, when I had my PSA jump early this year I had new scans done and
a small amount of free fluid was seen in the pelvis. As I worsened, and
in preparation for my radiation, another pelvic scan was done and there
was an increase in fluid. Shortly after this I developed rapid abdominal
distention, shortness of breath (you probably remember this from
pregnancy). This is seldom seen as part of prostate metastasis, so it
wasn't until real discomfort that I was heavily diuresed and had my
first paracentesis. About 450cc of bloody fluid was removed, and
prostate cancer cells were found. I have had two more taps, once last
week when I had my paralytic ileus and this week with my fever spike to
104.8. My girth is under control now, as I am responding well to
spironolactone and furosemide. I breathe easily and sort of forget the
ascites except as part of the whole.

If you want to pursue this with your doctor, he might suggest plain
abdominal X-rays. I believe the most productive study, before jumping to
the huge high prices, would be an ultrasound. It is this study that is
used to select a spot for the paracentesis needle, so is obviously
highly accurate and instructive. Tell your doctor that the question has
been causing chronic anxiety and that you would both be much more
comfortable (and less bother to him) if your husband could have an
ultrasound looking for ascites.
