tranplant of sural nerve

I am 41 years old, I was diagnosed  vith prostate cancer.I'm married without
children
PSA 11,15
Gleason score 8
T2b
Biopsy 6/6
After 4 cycles of chemio with taxolo, and estramustina pills to riduce my
prostate I'll be surged in 2 weeks.
At this moment the PSA is 0.18 and my prostate is just a little bit larger
than normal.
I'll be surged and during the radical prostactomy there will be a cavernous
nerve graft reconstruction using sural nerve grafts.
I'm italian and I'd like knowing if there is anybody that is surged in this
way.

Hello Stefano,
   Sorry for your troubles.  It seems with the reports we have here, that a
Radical Prostatectomy, may not be advantageous concerning the numbers given.
PSA of 11 is high.
Gleason of 8 is high
I am no Dr. and feel that the Dr.s you have are treating you well.
I believe the nerve graft is sometimes from a leg to help your erection....
Anyway, some Dr.s remove one or all nerves depending on cancer spread.
Erectile nerves.
Sometimes they replace those with grafts from leg muscle.....
In any event, you are under good care, and hopefully all the nerves (both)
will be spared, or one will be spared.
One can make you fine.....Loss of 2 can make you not fine....
Nerve graft can work!
I am not a Dr.  I have one nerve.  It works...I had RP 1999

There is about a 50% chance that a nerve graft will work- mine didn't.  My
nerve was removed from the calf of my right leg- the numbness is hardly
noticable now.

Your English is excellent.

There are few people on this newsgroup who caught prostate cancer at 41
years old.  I am extremely sorry for you.  I hope you were not hoping for
children.

Surgery is about the only alternative for a 41 year old and then only if he
is clear of cancer in his bones and lymph nodes.  Have they performed a bone
scan and CAT scan to make sure it has not already spread?

Otherwise, your age, PSA, Stage, and Gleason put you at moderate risk of
recurrence.  You will need to attack it aggressively and not give u until
you beat it or it beats you.

As to sural nerve transplants, there are a few here who have had the
transplant and who will probably comment.

You are the first I have heard of that had  chemo before RRP.  I had a couple
months of casodex tablets before radiation, but nothing before the RRP.  I have
always heard that radiation and chemo turn much of the prostate into scar
tissue and make surgery difficult.  Your stats sound much like mine when I was
diagnosed, but I have no idea what "surged" means.

Operated on??  He mentions being "surged and RP" in the same sentence.

Ah- mention in the same sentence usually implies that they are different.
"Surged" has the feel of a flushing-out, as I had to do the evening before my
colonoscopy.  Perhaps it -is- a mangled conversion of a noun into a past-tense
verb...

Grin.....you do have a point there. And I don't even speak Italian!!  Ron
has often said that having to be 'surged' with CitroMag and an enema before
each HDR treatment was high up on he *List of Worst Things*.

Nun, Ich sprache Deutsche ein bischen und Poot Passah Thai ging/dee mak, the
latter being a grossly over-polite claim of my Thai friends.  I should have put

Sounds Gernman to me.  Something about German language, actually.  But, I
can't quite make it out.

Stefano,
i had a seral nerve transplant done with my radical prostactomy in Jan
2002.
For the first 18 months, to achieve erections i only had success with
injections. Since thne i am able to have intercourse with meds (viagra
and levitra). Patience is a virtue.

I have no unpleasant after affects from the transplant in the foot
area -  occasional tingling is all. I am very active - cycling and
running, so it has not stopped me from doing those activities.

If you are planning to have children in the future, you should
consider making a deposit with a sperm bank, as the surgury will
eliminate your elaculate and a sperm bank seems less painful than
extracting sperm through other means after your surgery.
