He has vocal cord paralysis. Would this mean it's already spread and much

The variables are numerous, there isn't much info to go on, and only a
doctor should give an assessment of the patient's prognosis.
That said, there are lots of different types of lung cancer. Some spread
very quickly, some more slowly. Some are Primary lung cancer, other are
metasteses from other cancer elsewhere in the body. In general, though, lung
cancer in an advanced stage, where it is causing physical symptoms, has a
very poor prognosis for survival.
