Hi everyone,

I lurked here for a while back in October November last year, read a lot,
learnt a lot and also cried a lot over your personal cancer challenges. At
the time I thought posting about my stage 1-2 cervical cancer was really
nothing compared to what some of you are dealing with, so I let it be and
had the Letz procedure in November and thought that was it and time to get
on with life.

Bfore that first surgery I had argued with the surgeon over getting it
removed NOW or waiting 12-18 months to see how it develops as he wanted. I
told him that it was there now and I wanted it out of me now. He was rather
arrogant and dismissive but after talking to a friend who had it as well, I
think I did the right thing in wanting it gone. After the procedure, I
experienced no bleeding, no pain, absolutely no indications at all that I
had had anything done.

I had a repeat smear as a check up last week and now the results have come
back that I am stage 2 going on 3. My questions, can it come back this fast
and far in 4 months?  Is it more likely that they missed some the first
time? Have I got the right to request my medical records to see if this
surgeon actually did the procedure?

Although grateful that once again it has been caught early, I think having
it reoccur so soon after the first time isnt a good sign. I am assuming they
will want to do the same procedure as before but can I request more radical
treatment to ensure it definitely wont be coming back? ie. having a
hysterectomy or at least my cervix removed?  Im 35 and have two sons and the
fact that I will not bear any more children isnt a worry to me.

Thank you for any help in advance,

Best Wishes

Kim.

Hi Kim,

Letz is a mispelling (or alternative name for) of LEEP procedure
http://www.leep-procedure.com/
Abnormal cells are called dysplasia (cervical dysplasia)
If you were Stage 3 cervical cancer, somebody would be lighting a fire under
someone to get moving..

So I think you've got cervical dyplasia , grade 3.(which is kind of
pre-cancerous)

http://www.womenshealthchannel.com/cervicaldysplasia/treatment.shtml (the
grading is there under "staging")
The cone biopsy is usually performed for high-grade dysplasia, dysplasia that
recurs, and suspected cancer.

Gosh, am I sorry he's arrogant and dismissive.
At that point, though, you were a lower grade and he may have felt more complex
procedure/surgery not warranted.

Years ago, where I came from, they immediately did the cone biopsy with high
grade dysplasia.

The base of the uterus (neck of the pear) is a muscular wall called the cervix.
http://science.howstuffworks.com/human-reproduction4.htm
and seems to claim that has something to do with lubrication..
http://www.abbys-sexual-health.com/articles/about_my_vagina/about_my_cervix.php
(looks like that last lady also had problems talking about these organs with her
doctor).

There's a nurse in our Province who does a show on non-commercial TV channel,
called "Talking sex with Sue".
She picks subjects, explains anatomy, covers part of your issues, does demos
with props, people call in with questions. She's very good and covers a lot of
topics. People in every country should be so lucky to have someone like that.

Cone biopsy (and other) are also covered here
http://www.cancervic.org.au/cancer1/patients/cervical/cervical.htm (under
treatments).

A hysterctomy is a bigger surgery and then one gets into questions like, while
in there, should I remove my ovaries or not, (which has implications for
osteoporosis, mood, bladder function, possibly other things that one expects
during a "natural menopause", hormone replacement etc..), scarring, longer
recovery period, some can get what's called adhesions or other post-op
complications.

Is there a female gynae/surgeon that you could see?  Or a women's health clinic?
To talk things over a bit first?

If Dr. Dismissive has a secretary or nurse, can you call and ask him/her, if you
can pick up a copy of the report(s)?
Say you want to discuss the implications of each option, with a women's health
clinic and with your partner, then go back and discuss the options with the
doctor. Take the partner with you, to take notes and/or record the conversation.

Hopefully, you can work things out for the best option for you.
Keep in touch and let us know how things went, ok?
J
PS I'm hoping you'll read most of those websites, take some notes about the
procedures and implications of each decision and/or risks etc. I'm not sure
whether a cone biopsy can be done with just what's called an "hyponotic" as
opposed to general anesthetic (which carries some risk).

I hear it's sometimes difficult to get records in UK.
They sometimes get their backs up about it here in Canada.
I have all kinds of theories about why: paternalistic attitude towards patients;
too much fuss/cost to photocopy them; worried someone's saving the information
in case they later want to sue them; nuisance factor/don't have time; the
records aren't filed yet (sitting on a pile);  they worry that we'll get
stressed/worried about "medical terms" that we don't understand. That happens
too.

But to be honest, I've made 2 major moves in my life and I've lost a lot of
medical records, because I did not ask for them at the time. After I moved, I
sent for my " medical records".  I expected copies of lab reports, surgical
reports, pathology reports;, but no: all that was sent (to the new doctor) was a
2 paragraph summary that didn't even mention a lot of the above.
And then doctors here asked me questions to which I did not have the answers. So
it really is helpful to keep copies.

So absolutely, I think you have a right to copies of your files; if they ask
why, tell them so that IF you move (anywhere in the world), you don't have to
write them later, to pull out old, stored records, so that your new physician
can have a record of what was done (sometimes 10, 20, 30  years earlier).
Hopefully that will satisfy them.

Thank You J for taking the time to reply.  Thank you also for the
information.  I start the merry-go-round of doctors visits again in a couple
of weeks so this time I will be better informed before I go.

Very much appreciated.

Kim.

You're welcome, Kim and best wishes.
I'm sure you'll be fine but do let us know. :-)
