I have a medical mystery going on and I am not sure whether my cancer is
involved.  I am a survivor of adenoid cystic carcinoma of the breast.  A
normally slow growing and indolent cancer.  My original dx was in 1998 and I
have been an advocate ever since, so I have 'met' several women who have
also had this cancer.  I know of two ladies who have passed away from the
disease, who had liver and bone mets, on top of the usual lung mets common
to this cancer.  OK, enough of the history.

I had a great deal of pain in what I thought was my intestines a over a
month ago.  My PCP sent me to a gastro guy and he found nothing clinical to
cause it.  It did not last long, so we stopped checking.  In the meantime, I
was sent to my onco for a routine follow-up.  My blood tests came back with
elevated AST, ALT, ALK (liver function).  They told me not to be concerned,
but I was because I am sober as a judge and only take one medicine
(synthroid) at a very low dose.  Any blood tests that showed these in the
past, were actually on the low end.  In any event, I addressed it with my
PCP and he re-did the tests, along with GGT and Hepatitis panels.  All heps
were negative, but the GGT was high first time this was taken) and the ALK
was over double what it was on the original test (the week previous).  The
gastro doc had a CT of the abdomen done with barium contrast.  In the
report, it said the liver was fine.  I assume any mets would show.

I have a feeling that my original 'abdominal' pain was the liver, not the
stomach.  Although, according to that CT my liver is 'normal'.  I am not
sure if liver mets would affect these counts, without showing gross disease
on a scan.  Hope someone can help, or steer me in a direction to find out.

Some brief thoughts --.  Liver metastases are normally painless unless
advanced, so the negative scan and the fact that the pain went away should
be reassuring on that score.

Minor abnormalitites of LFTs are common, and not necessarily of serious
importance.  They are very  sensitive tests.

Have gallstones or a mild episode of pancreatitis been ruled out?    Exclude
drugs or herbs that have hepatotoxic potential.       I am sure your
gastroenterologist will be considering all these.

Thanks Peter.  The gastroenterologist is a new kid, fresh out of school.  He
has not requested any of the recurring tests.  It is my PCP that has.  My
ALK is at 225 and my GGT is at 210.  And I have not had a drink of any kind
in almost 20 years.  I take only Synthroid (mild dose) and liquid calcium
supplements.  The gastro guy said that these would not cause the increase.

My concern is that the onco did not take the increase seriously (said it was
a fluke) and they infused me with Aredia.  I held up really well until two
days ago.  Then I had a leg swell up like a balloon, my blood pressure is
spiking, and I have spent the last two days feeling intensely nauseous.
Everyone seems to be wanting to 'step away' from this.  My onco still
insists, after three consecutive high (getting higher) counts, that they are
all just 'flukes'.  My PCP says it is way over his expertise level.  And I
am afraid that my gastro guy may think that the GGT is about alcohol abuse
(even though I guarantee him that I do not EVER drink).

This nauseousness this weekend has me a little frightened that the Aredia
infusion may make a bad situation, worse.  Is it normal for a gastro doctor
to be treating a liver problem? or is there a doctor that specifically
treats unusual problems of the liver?

Hi Sharon,

I would suggest you ask your ONC for a CA-125 test and Transvaginal
Ultrasound.

Best wishes that they find the source of your pain.
