Can anyone send me a completed filled out copy of your FMLA form?

I was recently diagnoised with Stage III breast cancer and I am now in
the process of having my FMLA documents filled out. My job has denied
other employees FMLA packages so I just wanted to see someone elses
form to make sure eveything was filled out correctly. I am a single
mother and it is important that I keep my job is this economy. I must
start treatment immediately to have any type of survival rate. You can
cover up your name and doctor's name I just want to see how it is
filled out and what it says. I would really appreciate it

I find your request very odd.  I recently had a relative get her doctor
to fill out one for her for her job and it is the doctor who must give
the required information.  There are many questions about the reason for
why the employee is requesting to be on FMLA and what about the medical
problem would cause them to ask for FMLA leave.  Each person will
probably have different reasons according to what medical problem they
have.

Physicians seems to  be used to filling out these forms so they know
what is needed on the form.  If they leave something out they will
probably be contacted by FMLA and asked for the additional info.  My
relative did not have a problem with getting it approved but it did take
some time before the doctor had it ready since it was several pages.

I think companies are wary of employees getting on FMLA since it seems
some abuse the use of it.  I read in my research on it that the doctor
must resubmit the form every 6 months if the employee needs to stay on
FMLA.  It is also the employee's responsibility to request their doctor
to do so.  If it is not done, the approval is cancelled.   Also, if you
change jobs (which my relative did right after being approved), you
cannot take the approval from the former job with you.  You must get
your physician to do another request for you for the new job.  The
physicians must hate this because it is more paperwork for their office!    

If you really feel the need to be on FMLA, if you haven't done so
already, I suggest you read up on it and know exactly what you are
getting involved with and what your responsibilities are when you are
approved.
