I have had Asthma (diagnosed) for 8 years. I run about 3-4 days a week since
I was 17 years old. I am now 44. I had just used albuteral ( 2 puffs) just
for running, but for the last 2yrs  I started to use it more and more with
no effect on my ability to run more than a 1 mile ( I normally would run 4
miles). I started to user Advair 100/50 twice a day since March of this year
(2006) and it was a miracle drug. I was able to do the 4 miles with out
using my Albuteral inhaler at all and ran at a good pace. I have been in the
best condition in years, but recently ( for 2 weeks) I would get skipped
heart beats to the point of being uncomfortable and scary but no pain. I do
drink alot of coffee (6-8) cups a day. I have now for the last 3 days
reduced the dose to 1 inhalation of Advair in the morning and no coffee and
I have no

Skipped heart beats. I ran 4 miles today and was a little labored on the
breathing because I guessed I have reduced the dose of Advair. My question
after all this is, is my liver not getting ride of it fast enough for 2
doses or is it the Caffeine complicating the issue.

Thanks.

First, you should see a doctor about the heartbeats and possible drug
interactions.  I am not a medical doctor.

After that disclaimer I can say that skipped or delayed heart beats can
be from Atrial fibrillation or ectopic beats (not sure about spelling).
My son has the latter.  Generally, they are not dangerous, but can be
scary and occassionally cause a slight, momentary light-headedness. He
was told that caffine exacerbates the condition.  I have such beats,
too, usually not when I run, but at random infrequent moments during the
day.  My doctor had me wear a 24-hour monitor, but it showed nothing.
Other cardiac tests showed nothing.  But best to be safe.  Drug
interactions with caffine are possible as are other problems.  Have it
checked out.

On the up side, I'm happy to hear about your success with Advair. It's
great to be able to run again.  My miracle drug was/is Singulair.  I
could barely make a mile at a jog and I would have to stop and gasp for
air.  Now I can run at a better pace for 3 to 4 miles 4 times a week.
I'm 59.  That's a nice feeling.  Keep on runnin'.

In the past, Glaxo had warnings under Cardiovascular that stated:
"Advair can cause a change in the heart rhythm"
I recently looked and was unable to find these warnings (they could
still be there, I just didn't see them)  A-Fib, V-Fib, Asystole are all
changes in the heart rhythm.

Oh My,,, here it is! (line 9)

Important Information about ADVAIR
ADVAIR won't replace fast-acting inhalers for sudden symptoms and
should not be taken more than twice a day. ADVAIR contains salmeterol.
In patients with asthma, medicines like salmeterol may increase the
chance of asthma-related death. So ADVAIR is not for people whose
asthma is well controlled on another controller medicine. Talk to your
doctor about the risks and benefits of treating your asthma with
ADVAIR. If you are taking ADVAIR, see your doctor if your asthma does
not improve. Tell your doctor if you have a heart condition or high
blood pressure. Some people may experience increased blood pressure,
heart rate, or changes in heart rhythm. ADVAIR is for patients 4 years
and older. For patients 4 to 11 years old, ADVAIR 100/50 is for those
who have asthma symptoms while on an inhaled corticosteroid.

Complete Prescribing Information and Medication Guide for ADVAIR
DISKUS® (fluticasone propionate and salmeterol inhalation powder).

I would guess that the coffee has a lot to do with it.

You could try going up to Advair 250/50 at one or two puffs per day and stay
off the coffee (or at least reduce the dose.

I agree with Lou that you should see your doctor about it.
