hello
   have read this group since hubby (Bob) was dxed with RA over a year
ago..he is ok at the moment but gets a lot of numbness in his hands, also
says it feels like electric shocks...his wrists are all lumps and bumps and
the doc says he has carpel tunnel syndrome...is this a direct result of the
RA? or something completely different?   He is currently taking 1 plaquinel
200mg    and 2 meloxicam 7.5mgs     daily....but going back on the naproxen
soon  (Meloxicam not showing any improvement)....Our problem is that he
wants to rub in an ointment (Volteral) to see if this helps at all....but
reading the contra indications   it says not to use it with  nsaid     .
Has anyone used this cream with good effect or is it not strong enought for
severe pain on it's own?    Or can he use it with the drugs he's on?     Any
info much appreciated....

I had carpal tunnel problems back when I first started having RA type
problems.  I'm most grateful that the ortho guy helped to push through
insurance for the CT release surgery before anyone got to mess around with
treating it with medication

I'm not normally a big fan of surgery.  But this one is a piece of cake and
a quick and permanent fix to the problem.  Can't say that about many things.

I can't find Volteral listed at www.rxlist.com but I'm assuming it would
be an ointment form of Volteran which is and anti-inflammatory.  Two
such drugs should not be taken without consulting your doctor or
pharmacist.  They can excessively thin the blood and cause internal
bleeding.

Pam,
The carpal tunnel syndrome is really common with the RA. Basically the
wrists get inflammed and the compression causes the numbness etc.  Call it
what it is - if the inflammation from the RA goes down then quite often the
carpal tunnel improves.

Bob is on only one very mild dmard (disease modifying anti rheumatic drug).
This is the plaquenil.  I personally would recommend that he talks to his RD
and asks if his RA should be under better control.  Adding another dmard
would help this.  The lumps and bumps could very likely be RA nodules which
would mean he is not under great control.  The meloxicam and naproxen are
nsaids so that is going to reduce some inflammation but do little for the
disease process.  The volteral is another nsaids which is why they don't
want you taking it with the meloxicam or naproxen.  This could cause an
overdose of nsaids in your body.  Again check with your doctor - perhaps he
feels that the ointment would work better than adding naproxen.

It is important to get the RA under control.  Bottom line - it sounds like
it isn't.  I have a thing about adding another name to what we have - we
could easily all end up with lots of diseases - RA, carpel tunnel,
tendonitis but if it is all related to the RA might as well call it what it
is.

Good luck - send Bob our way if you want.  There are some non-medicinal
things that will help that carpal tunnel a bit - ice, wrist splints, resting
splints while sleeping (lovely things with velcro to hold them - yuck!), and
finally some surgical solutions (which finally helped me since my RA took a
long time to get sort of under control.)  Bottom line though in my opinion
is to get an opinion from his RD about the disease.

Pam,
I have AS (ankylosing spondylitis) which is very similiar in a lot of
ways to RA, (autoimmune arthritis). I agree with Kelly above because
before I was diagnosed, everyone originally thought that I had carpal
tunnel. I shuffled between docs and then finally had a nerve conduction
study done, and it showed that I didn't have ct at all.
Flash forward two and half years later, I'm diagnosed, on Enbrel,
methotrexate, sulfasalazine and some other drugs, and my wrists don't
bother me much at all anymore.
A lot of things can mimic carpal tunnel. I would definitely agree with
Kelly on this one-perhaps you could talk with the rheumatologist and
see about adding in another drug (DMARD) rather than just adding in an
NSAID (Naproxyn). Many times in RA, more than just one drug is
necessary to keep things under control.

Thank you all for your advice and information.Much appreciated.and we will
certainly   be talking to Bobs rheumatologist as soon as possible.........
