My wife (34 years old) has a very painful joint which she has now had for a
year. She has had blood tests, biopsy, ultra sound and x-rays but no
definite diagnosis here in London, UK. She is now in bad pain and is
desperate for a diagnosis so she can be put right. The dr's say they have
never seen such a case of inflammation in a single joint.
All advice appreciated.

Which joint? Is it swollen as well, or just very painful? Have they
tried a corticosteroid injection? Are any other joints painful?

It is the ankle. She has had steroid injections which made it worse.

Has she been tested for gout? Acid levels don't need to be that high to
cause trouble.

has she had an mri?
my RA started in my ankle and foot and stayed there for over a year
before i got a second swollen joint (a finger) which was followed
months later with a swollen wrist. anyhow, all tests including blood
and xrays were normal. i insisted on an mri and lo and behold, it
showed something weird growing in my foot that scared the bejeebers out
of all the docs i was seeing. had surgery to remove it and it was
"inflamed synovial tissue."
if she's already had an mri and it's normal, then i give up!

boy, this is a puzzler. i keep thinking insect bite or sting, possibly
because one's ankle is so close to the ground. but for it to be ongoing for
a year makes me question this idea and makes it sound absurd.
i used to get huge inflamed reactions, that lasted several weeks, to
mosquito bites, even when the insect was killed just as it had started. and
i also had the same kind of huge inflamed reaction, while sitting on the
lawn, to an ant bite, of all things. on a joint, these reactions could
have played havoc, and perhaps for a lot longer. i wonder if the
toxin/venom from an insect had been encapsulated/trapped in a sheath or
perhaps a bursa sac, whether it could cause that much inflammation for that
long.
i keep wondering what kinds of blood tests were done on your wife and
whether they had done a toxin/venom screening.

Cap,
I think you've had some good suggestions. Can they treat the pain,
etc. until they find out what is wrong? This may be the beginning of
something that's just not showing up yet. The gout was a good
suggestion also.
I hope she finds something soon that will relieve her discomfort. The
diagnostic period is so frustrating.

joint now, it will spread. Has she seen a rheumatologist?
