Enbrel/MTX/Colds/ and Sciatica

I have been off Enbrel now for two weeks.  During that time I have had one
of the worst colds and coughs of my life.  That is why my doc took me off
Enbrel.  I have also suffered from severe fatigue.  We don't know if it is
just coincidence or if it had anything to do with the Enbrol.  I am
continuing my MTX injections.

Last night we went to a wedding and while we had a good time and the
wedding was nice the hard chairs caused my Sciatica to flare badly.  I have
a severe tendency to get sciatic pain in my left "sitting bone" after a
session in hard chairs.  It lasts for days.  This is worse than my RA now.  
If it ain't one thing its another.

Bob, that "sitting bone" pain sure sounds like it's bursitis or
sacroiliitis.  Ask your doc about it.  Seems to me, nerve treatment and
inflammation treatments could be very different.  My bursitis and SI
were the bane of my existence, until I was prescribed with Enbrel.  That
stuff has turned my life around.  

Interesting, in that you have a lousy cough.  I developed a miserable
cough yesterday.  A deep, unproductive, burn-in-my-chest kind of cough.  
I'm hoping it resolves rather quickly.  I don't want to go off Enbrel.  
I'm flaring too much with my everything tendinitis right now.  :-(

Hi Bob,,,  You and I have been fighting the same problem but I have not cut
Enbrel completely out, only extended the time between shots to double the
time.   The lungs are slowly getting better and my RA is holding as is.
That would not be the case if I stopped Enbrel all together.   I found that
more exercise as the weather improves has helped.

Hi Folks.

I caught a cold on New Year's day and didn't shake it for a month. It
left me with a sinus infection which I haven't been able to get rid of
since. I got some antibiotics (doxycycline) a week ago, but the sinus
infection still has not gone, tho it does seem to have improved.
Mercifully I didn't get the cough which often goes with these things.

I missed a dose or two of enbrel, but am still taking it.

Bob - read my Overdue Update for the full story of my
cold/bronchitis/sinus infection and now I'm going on 4 weeks of IV
antibiotics. I've been off Enbrel since January 3rd!  My RA is
definately letting me know it's still lingering in my body.  I can't
help but wonder what the abx will do.  Kinda makes me think of all
those on abx therapy for their RA and wonder.....or maybe I should call
it wishfull thinking.

As far as the pain in your "sitting bone"  - I had a bout with that a
few years ago. It took 10 months of PT and three different diagosis to
get it right but Diane hit it on the head with SI or sacroiliitis.
First it was called bursitis, then it was called sacroilitits or SI.
After treatment for that I had significant improvement but by then I
also had a major spasm in a muscle called the psoas  muscle.  When I
had that spasm released by a very good physical therapist, it ended my
ten months of pain.  If you google "psoas" images you will get an idea
of where this muscle is. Spasm in it can cause sciati type pain as
well.  About a year ago, Arthrits Today had a short article with some
exercises to help ease SI pain. I don't know if they archive them
online or not. But maybe you have old issues!  I'll look tomorrow and
see if I can find it. I believe I saved it. If so, I will scan it and
send it to you in email. Too late to look now.

Hey, thanks for posting this Di.  Out of curiousity I looked it up and
realized that may be one of the areas in my lower back that gives me trouble,
especially after sitting or standing for long periods.  (I was born with a
really arched spine and have been subject to back aches all my life.)

I found this site http://www.yogajournal.com/practice/169_1.cfm and tried the
Standing Release they talk about.  Boy did I feel it on my right side and now
the whole are feels much better.  I have something new, and probably more
effective, to use when i start aching!

Thank you Diane and everyone else for your helpful replies.  This PSOAS or
whatever it is has been plaguing me for a couple of weeks now. You all have
encouraged me to continue to seek a solution.  I was beginning to think
there was no hope for that problem.  You have given me a lot to pursue and
I appreciate it.

In Arthritis Today March-April 2004 issue - I found the online archives
with a table of contents and it looks like this issue might be the one

There's been A LOT of that going around in northern Michigan, even among
normally healthy people.  It takes them forever to get well and they have the
cold symptoms and fatigue all along.  It may well just be the nature of the
bug you've got, hopefully, so it will all get better when the virus finally
decides to go on its merry way.

Ouch.  thank goodness you at least had a good time!  The pain afterwards is a
little easier to bear when it's in response to a fun time rather than a dull
time.

Nann and all.  I am up at 5am typing this because my hip, sacro, whatever
it is, pain, woke me up early.  I am thinkful that my other aches and pains
are eaased by the MTX.  Still not taking the Enbrel.  I see my RD on Monday
for our regular checkup and blood work.  My cold, respiratory problem is
better but I have been plagued with fatigue like at no time in my life.  I
have actually napped each day which is definitely, absolutely, NOT typical
for me.  I think my wife thinks I am dying.  Just kidding slightly, but she
expressed concern.  My fatigue and my hip, butt, sitting bone, sacro, pain,
whatever it is is really bothering me.  I am going to ask my RD to "shoot"
that joint or send me to PT.  I need relief.  Heating pad helps.  Hot
soaking baths help for a little while.  I think my lack of excercise due to
not having any energy have added to the problem.  Sorry for the rambling
but I tend to do this in the morning.

Just got back from my RA doc.  He put me back on the Enbrel/MTX combo.  He
said that some studies have shown that the combo does a good job of slowing
the progression of RA.  He said that the symptomatic improvement shown by
the combo as opposed to just MTX is not dramatic but the difference in
joint damage over time is pretty dramatic.  He wants me on the combo for
the long term benefits.

He diagnosed my hip and sciatic pain as just that.  A neurological problem
caused by some stenosis and a narrow sciatic notch in my pelvis.  He did
some neurological reaction tests and found a slight weakening on my left
leg as a result of the problem.  I am now scheduled to start some physical
therapy for it tomorrow and I am pleased about that.

Thanks Harv.  The first PT session went well.  My therapist is very
knowledgeable and good.  I did mostly stretches today and had some sort of
electro stimulation therapy through patches applied to my lower back.  It
really seemed to help.  I am eager to continue this program.

Two sessions now.  Yesterday was remarkably pain free but Barbara and I
keep our granddaughter alll day on Thursdays.  We enjoy having her so much
and have great fun with her but today I am sore and achey again.  I guess
she gives me as much of a work out as my PT sessions.  Just not as well
targeted to be beneficial.  

We discovered that much of my pain is associated with the point of
attachment of the piriformis muscle at the sacrum.  I found a web site that
does a good job of describing the condition.  It is http://www.rice.edu/
~jenky/sports/piri.html .  So, much of my PT is piriformis stretching to
try and elongate that muscle to free up some of the pressure on the sciatic
nerve.  It is such an odd thing that this important nerve is so tightly
packed within that sciatic opening.  Sitting definitely aggravates my
problem and so I pay for my favorite activities, driving and working on my
computer.  Oh well.
