Advice needed - FAST!

Hi - My Mom has been in the hospital again and they are calling me to ask
permission to put in a peripherally inserted central catheter - I need to
know the risks for an elderly person with recurrent infections of having a
central pic line. She has diabetes and is prone to blood clots. She's had
one infection after the other for almost a year now. And then he went into
the hospital with sepsis last Thursday and is going to be on IV abx for 15
more days when they send her home today or tomorrow.  I'm concerned that the
pic line is just going to be another site for infection. I'm also concerned
about the placement in the middle of the elbow being so uncomfortable and
annoying. And most of all I'm concerned about their sending her home the
same day they do it. HELP!

A pic line is not really uncomfortable or annoying. After a couple days, I
hardly knew mine was there. If it wasn't for the tape and dressing, I wouldn't
have known.

If she needs 15 days of IV abx, then a pic line is much better than an IV
change every 3 days. And the IV changes are much more likely to cause infection
than the pic.

Is your mother sound in mind? I am asking because I remember the docs were
hesitant about putting those in  elderly people who have Alzheimer's (sp?).
Pulling them out would be a concern if the person was not of sound mind.
If she is coherent then I do not see a problem. I was supposed to get one
last winter for my Remicade, but my veins decided to start working with the
nurses..lol.
As long as it is being cleaned as the docs instruct I do not see why it
would get infected.
Is she on wound care with her diabetes? I would ask the wound care team what
they think. They could also give you some advice.

Thank you everyone who answered. More details tomorrow. Just can't sit up
anymore. Mom got the pic line after about 4 hours of coaxing her over the
phone....shesh!  I was on the phone today from 11 til 5!

Thanks again to everyone for their advice and prayers for Mom. Today I
finally spoke with her Infectious Disease Dr and I ask him what we could do
about her constant infections. She's had one infection after another for the
past year. Names of which I can't even pronounce yet alone spell. UTI,
Bowel, and now Sepsis.  This time when she was admitted she had C-diff in
her bowel, three infections in her blood and a UTI  Two of the three
infections in her blood were MRSA (she's colonized now) and Enterococous
avian. They asked me if Mom lived with a bird.  And no, she doesn't. They
said it comes from bird poop! How the hell did she get that???  Fortunately
we caught it before it turned into septic shock.

So basically he said there was nothing we could do because of her weakened
immune system and diabetes, this was just how it was going to continue until
... and he just stopped talking. We both knew what he was saying. And in my
heart of hearts I've known this. I've been telling my husband that it is
either going to be sepsis or her kidneys that will be the cause of her death
the way things are going. They have to use such strong abx with these
infections that they damage her kidneys, which are in bad shape to begin
with from the diabetes. He's very careful how much and how long he gives
them to her but it's a fine line he's walking. She grows weaker with each
infection and she's lost so much weight this past year -- I'd say almost 40
lbs.

So this is very hard and very sad for me. Her mind is sharp again since they
took her off Zoloft and switched her to Wellbutrin. And her quality of life
is so poor. She did start reading again. I'm happy about that. I bought her
some large print books and she's reading again for the first time in 5
years!  She's read 4 books since April!  (Hey DCham --- are your books in
large print? I've been looking for them and didn't find them at the book
store or library. She would really like your books, I know)  So that's good.
But living in the nursing home is so boring for her.  And this crap with my
back hasn't made it any easier for me to care for her. I don't really get
out of the house except on Tue or Wed and that's when Wayne takes me to the
dr. In fact, I was is so much pain Tues I made him turn around and take me
home cause it was too painful to ride in the car.  Not fun. So I'm not
getting over to see her or take her out and except for a few visits from her
sister, she's very lonely. Well, Wayne and my son go when they can. She came
for dinner about three days before she went into the hospital. She was doing
so well! My brother was her and he and Wayne went to pick her up and brought
her over.  But two days later .... and she has sepsis and is in the
hospital!  It's amazing she gets sick so fast!

So thanks again for your speedy replies and for listening. I'm not online
too much cause it hurts to sit at the computer. Mayo is still doing the
second opinion. At least they found my films.

Oh Diane. I am so sorry that you are in so much pain and that your
mother is beginning to fail so badly. My father had infection after
infection and though recovering from each, he was always weaker than
before. My mother and the doctors all had knowledge of his DNR but his
heart was strong and so several years passed before the inevitable. It
is such a hard thing to witness.
Gentle soft hugs to you.
Is it possible that someone in the nursing home who does have a bird,
managed to pass that along to her? Anyone from nursing staff to cooks in
the kitchen. Her immune system is so weak now that probably just about
anything might set her off.
Glad the Mayo is still in your plans and really glad they now have your
films. :)

This sounds like my middle sister awhile back.  Seems she went
throught the same thing, and the MRSA was the most difficult.  She
believes she got it in the hospital.  Anyway, she did the pic line and
had no problems with it, and she and her 3 daughters became experts at
sterilization of everything at home, including using all the medical
grade antiseptic soaps and sanitizers.  I'm keeping you and your mom,
along with all the ASA family in my prayers.

BTW, my sister is much better these days and once again has her
strength and good spirits back.  Although she has Lupus and Spondy
along with chronic respiratory disease, she just fights one thing at a
time to make it through the day.  I think this attitude has kept her
with us this long.
