Hi all
Has anyone with joint replacements had a colonoscopy and did they need to
take the antibiotics before it. I am somewhat perturbed because the surgeon
that I consulted says that I don't need the antibiotics before it. I said
that I had a card from my orthopedic surgeon that said I did. I also
mentioned that the card said I needed it before dental appointments. He
said that that was not needed. Didn't ask to see the card. Just said that
he would only order before proceedure antibiotics for people with heart
valve replacements. I am somewhat upset about his attitude. Just wanted to
know other people's experience.

Are you on any other medications? I did not require antibiotics before
endoscopy/colonoscopy (I have two THRs). I do not require them before
dental appointments, either. I'm also immunosuppressed (aplastic anaemia
and cyclosporin). If I was having dental surgery, that's another matter.

Last year when I had my first one I asked the same question. They said no,
not needed, even though I took them for dental procedures for two years
following replacement. I don't know the reasoning.

With most dental work and surgery, they are working in a highly 'dirty' (as
in 'full of germs') environment and quite possibly an already existing
infection, and it is probable that you are going to bleed some.
With a colonoscopy, believe it or not, the only way you are going to get any
bleeding is if they perforate the colon, and if they do that you have
considerably more problems than worrying about your joint replacement
getting infected!
At least, that's what I think...and I'm not a doc or a nurse, just thinking
in type...

They do often take biopsies during colonoscopy.

True. They didn't during mine, so I didn't think of that. So...WHY don't
they require prophylactic antibiotics?
(and that is a procedure that I think I will skip if it is proposed again.
shudder...)

Perhaps because antibiotics can through off the natural balance of flora
in our bodies, paving the way for normal bacteria and fungi to reach
pathological numbers by removing a range of their competitors. Just like
you're not supposed to use the surgical antibiotic soap too soon before
or after your operation: you'll get skin infections.

I have to have them before dental appointments with my THR.

Call your orthopedic surgeon's office and ask to have the surgeon return
your phone call. Tell them why you need him to call and you actually
want to hear his answer from his mouth which is why you are asking for
him to call you personally. Tell them your concern for messing up his
work by another doctor's refusal to follow his orders. You can tell them
that even though the colonoscopy surgeon may be right, it conflicts with
what you know to be right and you really need the peace of mind.
Hope that helps.

Thank you. It is a long wait ( I'm in Canada) before I get the date.

Lorrie, call your original surgeon and have him call your doctor or give you
a new prescription for the antibiotic if he thinks you need it for the
colonoscopy. I had a TKR, and I have to take antibiotics EVERY time before
I go to the dentist, and that was 5 years ago. You always need to listen to
the surgeon who did the joint replacement. I guess some orthopedic surgeons
say they aren't necessary after 2-3 years, but mine says "life". Good luck.
I found the colonoscopy to be better than I had anticipated, as long as they
give you something to help you relax. Nanny

Thank you Nanny. I have a card that says I'm supposed to have antibiotics
before dental work and possible infections risk. I think I may talk to my
family Dr. as I have an appointment with him next week. This surgeon who
will do the colonoscopy, seems to an "I'm God" complex.

Oh, one of *those*, huh? It would be nice if more doctors could work
together rather than wanting to have a one-man show. Nanny

Lorrie,
I had a TKR and two THR and my doctor ordered the antibiotic before the
colonoscopy and double checked to make sure I received it before he
began the procedure. Perhaps he was being super cautious, but he felt
the antibiotic was important and necessary.
I hope all goes well for you. I found the procedure itself was much more
tolerable than I had expected.

Thank you. I've had two hips, two knees replaced. I'm not afraid of the
procedure. Just worried about the antibiotic problem. Seems some people
have had it done without AB's, so I'll asked my family doctor when I see him
to-day.
