Mother had her tkr

RoseB - 22 May 2004 19:43 GMT My mother had her other tkr yesterday, I have simply been too busy to psot about it. My grand nephew is graduating this weeken and my sister and her husband have come. So with my coursework for the spring term, the grad, the company, and the surgery I have not had time for much. She made it through the surg. fine. She had a tkr in Sept, then a hip fracture and partial thr in January, and now the knee. It is a lot to go through, but at her age could mean a very long recovery. She is 77. We have also been dealing with some other issues, but I think those may have had something to do with stress, pain, and pain medications. That is the medical opinion for now.

debbie m. - 23 May 2004 17:41 GMT Glad your mom is doing alright. My dad is 86 and broke his hip had surgery w/pins then a year later had THR. He is doing fine. He went through some down times after surgery where he did not respond well. He seemed confused, etc. They say it takes a while for the anethesia to leave the body especially in an older patient. It seem quite a while before he was back to his self.

RoseB - 24 May 2004 05:40 GMT We went to see her today, since we could not go yesterday with the Grad dinner. The nurse informed us that yeaterday she was confused, ripperd out her IV, and was fighting the nursing staff. She crawled or tried to crawl over the guardrails, so they had to give her an injection to calm her down. I am not sure if it is the anaesthetic, or the pain meds that cause this problem. She did not have confusion with the two previous surgeries, but she did have major confusion with her first tkr. It seemed worse that time as it lasted about 4 days. This was a one day occurrence, as she was cogizant today of everything and did not seem confused.
It is dificult for us to handle, but we are prepared just in case. It is not that unusual for seniors, and is something families should know about if they have an elder loved one undergoing surgery.
Also her nurse had a major attitude and was forcing her to do things that she was not wanting to do. She was in pain, so was reluctant to position herself certain ways. The nurse's attitude made me feel anxious, and my sister too. My mother does not respond well when people are really demanding and forceful. At one time she told her "Don;t push me!" when she was trying to force her to bend her knee. Whether the nurse had some residual anger over the fact that mom was difficult yesterday, or not, I don't know. I do n ot feel she was treating her appropriately and we will be lodging a complaint. (My sister is a nurse, and she was upset too). This is my mothers 4th joint replacement, so she does know the ropes.
On the way home we passed an accident about 500 meters away from where my nephew was killed.

My sister did call last night and spoke with the night shift. She found out that they had given her a general rather than a spinal and that is why she was confused. She did mention that we were concerned about the care she was receiving; that we felt the nurse was unnecessarily rough. My mother has a foot that basically flops over and points to the side, she has not been able to point it straight for some time. Whether that is due to muscle weakness or nerve damage I do not know, but the nurse was yelling at her several times to straighten her foot. At one point when the curtains were pulled we heard mom say to the nurse "Don;t push me!" Hopefully the fact that my sister called will make a difference.
I don't know if there is a limit to the number of times you can have a spinal block or not. At one time there was, so perhaps that is why they gave her a general instead. At any rate, we would like to have known. My sister had spoken to the anaesthesiologist prior to mom's surgery and told him that she had a bad reaction from general, but she said she felt he was not listening to her. Obviously he was not, since he used a general anyway. If they had to use a general, we would like to have known so that we could be prepared, but also they needed to be aware that it would make her confused. The confusion is especially a problem because she rips out her IVs and tries to jump over the side rails of the bed. Since she is not aware of pain, she could get up and walk as she did with her first tkr.

I did not go today, but my sister did. She called me and said that my mother is much better today. I would still like to know if there is a medical reason that they gave her a general rather than a spinal given her previous history with the general. Caroline Marold - 26 May 2004 05:21 GMT Rose, the last surgery I had, the anesthesiologist asked me what I wanted and then told me with my medical history, kinds of drugs and number of surgeries I had had, he felt more comfortable giving me a general. He had some very valid reasons but that was before the anesthesia and only the good Lord knows what they were. Sorry I can't remember what they were for you but after he listed them, I agreed to the general. Might have been something to do with the heart even though I don't have heart problems unless you count high blood pressure. Sorry, so vague. Duckie

I think it helped that my sister had called the night before, because she seemed to be getting better care and pain relief.

RoseB - 27 May 2004 06:54 GMT Well, I mentioned about my mother having the grouchy nurse. When that nurse had taken her to the washroom she was not aboe to void. My sister noticed, the next day when she went to see her, that she was still unable to go. Yesterday when she called she found out that Mom's potassium levels were very high, and one kidney had shut down.
She is now on IV fluids, and has seen a cardio. Hopefully they can figure out what is going on. I am wondering if her body is reacting to having three surgeries in a short period of time. It is different when you are older, for sure.
