RoseB - 23 Apr 2004 04:48 GMT And of course today was feeling fine, and asymptomatic as compared with the problems I had last week. I took the pictures that I had taken by digital camera, even though the color was not accurate and he was not able to see the intense redness that develops over my foot when I am having "the evil foot thing."
From the pictures, he does not think that this is vascular but instead some sort of very acute condition related to RA. He did recognise that it looks very painful because the swelling is so acute it looks like the foot could burst. Cindy ( I hope I got the name right) in talking about her acute episodes with gout, describes a similar thing, but I do not have high uric acid levels so gout is ruled out.
He said that without the RA it would look like one of the fevers (he named it..I forgot it) present in African countries that is highly infective.
At first, he indicated that he had never seen anything like it, but then said he had seen it once but it was not as acute nor did it recur as often as what I have experienced. So diagnosis for the most part is unknown except that it seems to be an acute flare associated with my long standing RA, and seems to originate in the tendons.
He did say that I can not go on like this as it is too painful, damaging, and disruptive to my life. This is especially so since it has increased in frequency over the past year.
So it could mean that remicade is not doing it for me anymore. We talked habout switching to humira, however that has not yet been appoved by our pharmacare plan.
The options that we looked at included going on prednisone continuously, which is a bad idea for me considering I already have osteoporosis, or trying colchisine, which is used to prevent acute inflammatory attacks and not just for gout. Colchisine, unfortunately has the side effect of diarrea. I thought that I would try that, as if it helps, it seems to carry less risk for me than the prednisone would.
He did keep my pictures, some of which will be placed in my file, but he indicated that they could also be used at conferences. Argh!! I guess if it helps someone I don't mind- it isn't as if anyone will say, "Hey aren't those Rose's feet?" LOL
So I will start taking the colchisine and hope that it helps.

Don't you sometimes wish you could see the doctor every day just to keep your symptoms scared away? Hopeful for the colchisine for you. If diarrhea becomes a problem, check out that med they talked about in the "new Arava user" thread. It's helping folks with diarrhea side effects from Arave, so I bet it'd do the same for you.

Iti s interesting that yesterday my foot was "normal" whereas today it is red and puffy- not to the extent it is when the evil foot thing is present, but puffy enough so you can tell.
As an aside, the other Grade One teacher in our school came into my classroom this morning, and said she thought she was having a heart attack- but she did not want to go to ER, in case it was nothing and she would feel silly. I basically told her so, "Feel silly" and I'm driving you there right now. Fortunately the principal was at school already, so he took her. Her BP was very high and she had arythmia. Although they don't think it is a heart attack per se, but rather unsstable angina, they are taking it very seriously and are transporting her by ambulance to the city tomorrow so that they can perform various tests.
I feel really good that I convinced her to go, as she was pretty adamant that she wasn't going. She really had all the symptoms- numbness and tingling in the face and right arm, a heavy chest, and shortness of breath.
They actually told her how pleased they were that she did come in early before any damage was done. Its a good lesson for all of us really, to not take these things too lightly. Rose @}>-->>> Please remove "Ima" to reply privately!

I take it with my evening meal with the Bentyl which gives the drug all night to work. Then usually, I can get up, eat my breakfast and what ever is going to happen does so first thing before I leave the house. Duckie

Rose, really, really praying that this new med does the trick for you as you have been dealing with this foot thing for far too long. You definitely deserve a break from it all! Praying also that the side effects of the new med will not be nearly as bad as you are anticipating them to be!!! Let us know how you make out with it all!

I am not yet giving up on it, but we will see what will happen over the next few months. I would rather go on humira than enbrel, for some reason, but that has not yet been apporved here. I think Diane C is at 7mg per kg of body weight at 8 weeks. Not sure what MaryZ is taking but she is using MTX with it to hold things stable. Are you taking MTX with your Remicade? Duckie

I am at 5 mg per Kg of weight and fortunately I have been able to drop my mtx down to 22 mgs. The dose increase did the trick for me and we have never needed to increase it again. -- MZ

It really depends on what kind of relationship a person has with their doctor. I do not get any kind of hhhhmmms from my RD and he knows I expect a little talk around when I bring up something said or done in ASA. I have even handed him printed conversation or printouts of the latest stuff on a medicine or methods and he is either up on it or not and he will say so. He knows that I am not criticizing him in any way and just want a partnership with my disease. Now all the above does not work at all with an RDs office or if the RD is insecure with how to deal with a patient. In my world, Harv

His hmmm is his thinking out loud about changing how he looked at ceilings for dosage on Remicade. So it was a good share. I just love this guy. I got a light bulb flash look in his eyes and a hmmmm. Asked him if he wanted your RD's name so they could talk. He said it wouldn't be necessary.
