That is for someone without a valid placard from their home state.
California Vehicle Code:
22511.5(b) A disabled person or disabled veteran is allow to

"Travel parking placard for nonresidents who plan to travel in
California and are a disabled person or disabled veteran. Valid for up
to 90 days or the date noted by your physician on the application,
whichever is less."
you either mail in the form, or take your hang tag in for the 90 temp
one.
