Modified-Release Prednisone May Ease Rheumatoid Arthritis    



THURSDAY, Jan. 17 (HealthDay News) A modified-release form of the
steroid prednisone is better than the standard, immediate-release
version at reducing morning joint stiffness in people with rheumatoid
arthritis, a new study finds.

German researchers at Charite University Medicine Berlin studied 288
people with active rheumatoid arthritis. Half took standard prednisone
(a glucocorticoid) when they woke in the morning and half took
modified-release prednisone at bedtime. The modified-release tablets
dispense prednisone four hours after ingestion.

After 12 weeks of treatment, patients taking the modified-release
version experienced an average of 44 minutes less morning stiffness
per day than at the start of the study. That was 29.2 minutes less
than those who took the standard version, the team noted.

Their report is published in the Jan. 19 issue of The Lancet.

Both versions have nearly identical total drug exposure and maximum
concentration values. They have the same safety profiles.

"Our results have confirmed that the new modified-release formulation
is clinically and statistically better than the conventional
immediate-release preparation with regard to morning stiffness of the
joints. Furthermore, the effects of the new tablet taken at night were
achieved in addition to the established clinical control of the
disease resulting from previous treatment with conventional
immediate-release prednisone," the study authors concluded.

The study was supported by drug maker Merck, which markets the
modified-release form of prednisone under the brand name Lodotra.

Thanks Rose - going to keep an eye open for more information to come on
this.  For those of us who never seem to get off the darn drug completely
this seems like an answer.

This sounded interesting to me from the perspective of those of us who
have severe stiffness upon rising. It may provide the extra boost to
help us out of bed.

So glad Rose - been meaning to do it for so long.  The rituxan treatments
just put me back so much.

I am well enough though and have energy as of yesterday to go to Vancouver
tomorrow and then Seattle for the week with my mom and sister.  We are
babysitting my 5 year old nephew.  In other words helping out the Seattle
economy by shopping while he is in school, drinking wine (went off my mtx a
month ago to give my immune system a break), and eating anything that is
quick to cook and easy.  Buying cheese of course to bring home (expensive
here).  And need some new clothes.  Since the middle of November I have lost
over 12 pounds with reducing the first bit of prednisone and careful eating.
Exercises start in earnest in February after I am over the worst of the
rituxan.  Looking forward to them - my balance is so bad mainly due to my
feet.  Have to see if the rehab doctor will maybe inject some cortisone in
the balls of my feet - they are so sore now that the swelling and edema is
down a bit.

On the lousy part I am sure I have another uti even though I am on the long
term antibiotic.  Going to try to flush it out with lots and lots of water
and see if I can get rid of it prompto.  Seems when the immune system is
down so much with the rituxan I am a sitting duck.  It is getting old
though!  Might need another appointment with the urologist. (of course he is
on a two month leave so.....)

Anyhow off to pack.  Thanks for the information - I agree it sounded great!
Would help to not have that morning stiffness although the rituxan was
reducing it a lot.  Will see what happens when I drop the next bit of
prednisone.

Take care and remember that sick days are there for a reason - worried about
you!  Will be hoping that the rd has some answers other than 50 mg of
prednisone and painkillers.

Congratulations on the weight loss Kelly. That is wonderful news. Have
fun clothes shopping! Have fun drining wine and buying cheese, even.

Got several pairs of pants and 8 shirts.  Paid $9 each for the pants and no
more than $15.00 for shirts (that was a Liz Clairborne that is wonderful!)
Most were brand names 50% off already discounted price.  2 new pj's and lots
of new socks.  Blanket for the bed which was a great price.  Drank a bit of
wine but it still doesn't agree with my meds even though I was off mtx so
really only had a small glass a couple of days.  Bought way too much
cheese - US prices on cheese are so good.  Good thing thought that I don't
eat that much of it.  We cooked dinner each night (well my sister did the
tough part) and ate salads with each meal - huge ones with the leftovers of
fish and meat on alternate days.  Had tiny oysters and salad for mom's
birthday dinner.  Was great.

Oh and Kyle was good too but we ended up with a snow day and school was
cancelled the main day we intended to go shopping without him.  I did go to
123 bounce which is a huge blow up ball, bouncing place for a class birthday
party.  Too many kids, parents, babies etc.  What a day.  I finally
retreated to a soft chair and read my book.  It was a birthday party for
twins who are in his kindergarten class.

He was a brat this time - well not all the time but a bit of the time.  That
cheeky kindergarten stuff combined with overtired one day and a huge 2 hour
temper tantrum - screaming, lots of time out in the bedroom- mean aunt
Kelly.  Of course in between he would come out and give all of us hugs and
tell us he was sorry.  15 minutes later - meltdown.  Way too tired.  He goes
to kindergarten 9 - 4 which includes chinese school in there somewhere.  It
is more like grade 2 work - including about 20 minutes of homework a night.
(We sort of did it - really the parents work hard at it.  Don't agree with
it but i am not his parent.).  Anyhow there were great times with him.

The best part though was the shoe shopping.  Took me 15 minutes and I had
2 - yup 2 pairs and the guy actually understood my foot problem!  Wow!!!!
The entire store was wide shoes - over a DD.  It is the only store of its
kind in Washington State and you can believe Canada doesn't have one!  I got
two pairs of New Balance (one for the garden) for $85.00 and 109.00 both 7
1/2 4E.  They fit like a dream and with a temporary store bought orthotic
for $35.00 that he measured specially for me with a metatarsal pad in my
feet are almost painfree - well 50% anyhow.  Lots of dress shoes etc too.
Going to eventually go back down but these are a great help. Shoes have been
a major issue lately.

It was great talking to you Rose - you have helped me so many times over the
years.  Always nice to talk to someone who understands and believe me I can
sympathize with where you are at right now.  Med changes and disease changes
are always so scary.  And 50 mg of prednisone is just not fair - it is
cruel.

wow!  what a trip and what a haul!  You more than deserved to have a fun
break and I'm glad you got it, despite young kid tantrums.  I can't believe
they are so overloading kids so early in life.  Frankly, most of us did just
fine not having homework until 3rd grade!

those designer outlet places, and I can do real damage. LOL

I empathise re: Kyle. Tantrums are not fun for the listener. Two hours
of it would be excruciating. ;)

To be fair the entire 2 hours were not screaming. Part of the time was hugs
and I am sorry and 10 minutes later the pouting and eventually the crying
would start again. He was just exhausted the one day - I know that.  It is
just not his usual behavior.

We had a great trip though. I came out okay and then the day after went on
our annual quilting retreat at a friend's house.  We went Friday night and
stopped at 3 today (Sunday)  Almost finished a queen size quilt top - have
some more work to do this week if I get to it.  It was a blast but wayyyyy
too much chocolate and wine.  Have to get my stomach back on track this
week.  Good friends though.

Congrats on the weight loss :-))  Check out places like Goodwill and
Value Village (if you have them near you). I've done very well in those
two places. I'm only buying enough stuff to get me through the month as
I am going down a size every month and a half or so. And now that I'm in
the diabetes program and workout 3X a week with a trainer, the inches
are going in faster. The jeans I got at Goodwill in January actually zip
right off fresh out of the dryer :))) And the legs are getting very
loose. I only paid $4 for them, so it's cool and they will get them back
in a couple of weeks :-)

Hi Carole,
I regularly check the thrift stores but we just don't have as many large
sizes as they do down in the states.  Also must admit the prices of the
items I got there were better than thrift store - Ross's, TJMaxx, Nordstrom
Rack, Marshalls.  Did well at all of them.  Bought a velour sweat suit for
$15.00 to wear to and from the gym (I have a membership as of tomorrow and a
personal trainer to book for next week.  You are my inspiration.

Hope that trainer works out for you.  It is so impressive - have you been to
your cardiologist yet?

Hi Kelly,

My personal trainer is WONDERFUL!!  He's so much fun and he's giving me
extra exercises to do to help with my ice skating once I lose enough
weight to go back to it :)

We have LOTS of thrift shopes here in Bellingham, and also a number of
consignment shops. I have a bunch of stuff that I'm going to take in
soon. I'm hoping to get some $$ for it so I can buy other stuff :))

I see the cardiologist on February 14th (how appropriate is that?? :-))
I can't wait as he hasn't seen me since last August!

I"m glad I'm inspiring you and others. It's the reason I put my new
website up. I am hoping that others will read what I have written and it
will help them.

I got a scholarship to the "Y" so I'm going there to join today. We have
a pool here at the apartment complex, but it's small. It was fine when I
first went back to swimming, but now I'm ready for something longer. So
I'm going to go there. And when the university is on spring break, I'll
still be able to workout as they have the same machines there as the
university has. My trainer is going to give me a list of all my seat
settings on the equipment and the weights I'm doing on each so that I
can keep going :))  By the end of the summer, Hugh Hefner is going to be
phoning wanting my photo! LOL!!!

Let me know how you do with your trainer. I hope you get someone as
wonderful as mine! :-)

Thanks Jo.  I pushed the fluids and think I have avoided the darn thing.
These infusions just wipe me out.  Thank goodness might not need them for
another 10 months to a year (this is my projection anyhow).

Off to bed early - feeling dragged out today.  The 6 days away was really
too much even though I took it easy.

interesting.  my mom has chronic polymyalgia and found that she is much
better in the morning AND sleeps much better if she takes her pred at night.  
i think i'll forward this to her - it might be an even more effective option,
though she's doing well enough with the current plan that she may not want to
try it.
