Going to see my RD on Monday and I want to be knowledgeable about pain
patches. (And right now, I'm not.) I need something for my shoulders. I'm
in 24/7 pain and am getting very tired of it. I'm on Methotrexate, 8 pills
weekly, plaquinil, lodine, folic acid and darvocet or vicodin at night. I
also have a Rx for prednisone but I'm afraid to take it. The last thing I
need is to gain weight. Also, the doc said if I used it for more than 8
days I would have to back off and not just stop taking it. ??? Is that
right?
Forgot to mention that I have both RA and OA.
I've read on here about Ultram 300 ER and that some of you are having
success with it. Do you take it along with other drugs?
And the patches -- what kind do you use and what strength? Do they stay on
for a certain number of hours?
Big thanks in advance for any guidance. I want to be well armed when I see
the doc on Mon.

Jean -
Back off means taper off. For instance, if you're on 10 mg a day, your doc
would have you take 9 mg for a week, then 8 mg for a week, etc., until you
didn't take it anymore. I wouldn't be afraid of it at all. Weight gain is
not mandatory. The prednisone just makes you very hungry. I find drinking
lots of water keeps me feeling full so I don't eat so much. (Of course, I do
run to the bathroom a lot. :-P)
As for the patches, they're one size fits all. It is about 3.5 by 5.5
(larger than a 3X5 index card, but smaller than a 4X6 card). You can use up
to three at a time, 12 hours on, 12 hours off. They are Lidocaine (or
Lidoderm), depending on brand name or generic. I swear by them because I can
put one on my shoulder and one on my spine and still have one more for
wherever it's needed most that day. For me they really work well and I love
my RD for prescribing them!

i'm interested in people's responses to your questions as well. i asked
my RD about the pain patches and she said she doesn't use them with her
patients. i may press her on it. she prescribed ultram for me, but it
seems ridiculous to me to take something that eases pain in my whole
system when it's only one lousy spot on my ankle that i really need it
for. i am not sure why she won't prescribe it. she said it would be
bulky under my brace. are the patches bulky?? i think not.

They're about as thick as a wash cloth or a woolen sock (I'm
guessing).
They are COLD when applied. <g>
I prefer to use the TENS unit on my shoulders, the patches on my ribs,
hip and leg.
My patches are labled:
NDC 63481-687-01
ENDO Pharmaceuticals
Lidoderm (r) Lidocaine Patch 5%
Rx only
Each adhesive patch contains:
Lidocaine......700 mg (50 mg per gram
adhesive) in an aqueous base.
DIRECTIONS FOR USE
(Do not sotre patch outside the sealed envelope)
Cut the envelope along the dotted line.
Before next step, a patch may be cut
to fit the area to be covered.
Remove the transparent release liner (clear plastic
backing) before application of patch to the skin.
Apply up to three (3) LIDODERM(r) patches at one
time to cover the most painful area. Apply patches
only once for up to 12 hours in a 24-hour period
(12 hours on and 12 hours off).
WARNING: Keep used and unused patches away children and pets.
... URA Redneck if you entertain yurself for an hour with a flyswatter.

JEAN,
I notice that the pain patch replies so far are referring to the
Lidorerm-Lidocane patch. I have RA, osteopenia, spinal stenosis
and other RA related problems. My RD just refilled my Lidoderm
patches as he is a firm believer that pain reduction is essential
for quality of life.
There is another pain patch which has not been discussed yet and
that is the narcotic pain patch --- the Fentanyl patch which I also
use in conjunction with the Lidoderm patch. The Fentanyl patch is
small and a very thin plastic coated with the pain medication. The
very narrow outer edge of the patch has an adhesive that adheres to your
skin somewhat like adhesive tape. I wear mine for three days (72 hours
limit) and then change out for a new one. These patches are in
increments of 25. I started at 25MGH and am now at 50MGH. Some RDs
shorten the days to 2 days (24 hours) between changes when the patient
wants better pain control. Others increase the strength of the patch.
Because of a current spine problem which is a herniated disc my RD has
also allowed me to use my oxycodone 40MG ER tabs every 12 hours and
my 5 mg oxycodone to handle break through pain every 4 to 6 hours as
well. These can cause dizziness, nausea, sleepiness and constipation.
It also requires a good sense of when and how to use them
There are also heat patches and cold patches to help handle pain.
I sometimes use my gel pacs which can be heated in the microwave or
frozen in the freezer (which ever works for you) can also be used.
Cold does more for pain for me.
I have an appointment with a neurosurgeon to address the herniated disc
problem.

The brand name is Duragesic... they're prescribed 25mcg/h, 50mcg/h, etc.
There's a big jump in going from Darvocet and a few Vicodin to the Duragesic
patch. At one point, I was taking 12 Norco a day (10mg hydrocodone/325mg
apap) The tylenol is was gives the ceiling... too much tylenol will kill
your liver.
There are so many other pain meds to try before Duragesic- even adding
non-pain specific drugs such as anti-depressant, muscle relaxer, etc can
help some.
Sounds like you need to find a doctor who understands pain and isn't afraid
to aggressively treat it. All the RDs I went to, were afraid of pain. They
were willing to give me Elavil and steroids but that's it.
