It's back: The evil foot thing

About a month ago I had the evil foot thing. Doc gave me 50 mgs of
pred, and i took it for a while, but did not want to do the full
course because I have been on high dose pred so much in the past five
years. Because I stopped the pred early, the evil foot thing
reappeared in the other foot. I did sort of ride it out, and was doing
ok.

Then on  Monday night I developed an intensely itchy area on the top
of both feet. Ulcerated areas developed and they were weeping a clear
yellow fluid. Today I developed the pain and swelling that
characterize the evil foot thing. I saw the doc, and he said he could
do a biopsy but it would probably not prove anything. This now looks
more like the pics you see of vasculitis with the open oozing wounds.

I will take the pred and use polysporin so that it does not get
infected.

I don't feel well- fever, achy, generalized malaise.

Doc suggested I skip today's enbrel.

I am a tad upset.

(I am not going to give up my studies though. I will try to lighten my
load in other ways but I am enjoying my course work too much and am so
looking forward to going to Baltimore.)

Rose, so sorry that thang is back again.  Looks as if it just loves to hang
around and bother you.  It sounds terrible; surely hope it won't last long.

Because I have never had the open ulcerations before, I do not know
what to expect from that part of it. In general this seems to take
about two weeks, but with pred it starts improving right away. Without
pred, it is one of the more painful things that I have experienced.

rose, i'm so sorry. this sounds miserable. does your doc know about the
fever etc? i'm wondering if you should be on antibiotics.

It is just part of the inflammatory process that makes up this whole
package. I have always wondered if there was an  infectious component
to this thing, but the docs do not think so.

Rose,

That does sound nasty, no doubt about it.
I hope that it clears up quickly for you so you can get back to the
Enbrel.

Sounds very  uncomfortable, Rose.  I hope you sort it out, and feel
better soon!

I feel better pretty quickly on pred. I should have stayed home from
work, but I am doing end of year reading tests and i need to keep to a
schedule. I don't feel quite as "ill" as i did yesterday.My feet are
still red and swollen, but not nearly what they were yesterday. The
ulcerated areas have sealed over to a degree.

I went and boutght some 5 pound hand weights after school. The as I
was taking the groceries out of the trunk I managed to drop the weight
on my foot from about chest level. Toes are getting a little bruised
but I put an ice pack on right away, and took some T3.

I am just thankful that it was not a 10 lb weight.

Oh Rose!  What you really didn't need was to have that weight dropped on
your sore foot.  I am so sorry for this additional trouble.  Will be praying
for healing for your feet.

talk about looking on the bright side!  Your spirit is incredible.  I hope
there aren't too many ramifications of you tossing around that hand weight.

Rose it sounds like you have Cellulitis this is an acute bacterial
infection of the skin and subcutaneous tissue, this most often occurs
in the lower extremities   You stated that you are taking Enbrel, that
lowers you immune system and leaves you wide open for infections, If
your doctor has not yet ordered a blood culture then you should request
one.

This is something that is recurring, and was happening regularly while
I was on remicade. After I switched to enbrel, I did not have a
reoccurence for a year. Cellulitis was ruled out before. My rheumy,
after seeing pictures thought it looked like some mediterranean fever
if one took out the RA. It is one of those "mystery" things that no
one has pinned down an actual diagnosis for. Now though with the open
ulcers it does look more like vasculitis.

Did they think that maybe you are allergic to something in the
remicade? I know that enbrel slows down your immune system that could
be why it gets so bad. Are you diabetic? Well I hope you find out what
it is and are able to get rid of it good luck! I am new to the group
and knida about a year into arthritis and Fibromyagia at least knowing
what other people are going through I don't feel like I am going crazy
that these aches and pains are actually real.

It would be a rare or unusual side effect as opposed to an allergy. I
am not diabetic.If it is vasculitis, it is vasculitis that is
associated with RA and may simply mean that there is another
associated condition. My ulcers were small in comparison with the pics
you see when you do a search, but having them there means that perhaps
that is the real diagnosis.

I have had long standing RA so over time it likes to invite some
friends.

Ye, the pred takes it down fast, and applying polysporin three times a
day has really helped.

Thanks Denise.

Also, we had a day off Friday, then it was the long Victoria Day
weekend so I was able to stay off my feet.

Well shoot, Rose,  I am sorry to hear this!!!

Sure hope you aren't now developing a reaction to the Enbrel or to all
of the biologics!!!    Major bummer!!!

Thank, Donna G. That is what I am afraid of as well. I do not want to
burn any more bridges. I have done so well on enbrel otherwise.
