there is one exception that i know of, it was contained in a medicare
document concerning the state of iowa and a ruling made in '96.
covers ' custom molded space shoes' for" rheumatoid arthritis, congenital
defects and deformities, neurotropic, diabetic and ischemic intractable
ulceation and deformities due to injuries"
