In 40% of treated patients, an arthralgia- myalgia syndrome develops. The purpose of this randomized, double- blind, prospective study was to investigate whether intravenous (IV) administration of methylprednisolone (MP) prevents this complication. Sixty-five patients, 34 women and 31 men, ages 36 to 80 years, received either normal saline before and after TDI (group 1), 125 mg IV MP before and saline after TDI (group 2), or 125 mg IV MP before and after TDI (group 3). Patients were observed for 72 hours and reactions were recorded and graded according to severity. Fifty-eight percent of group 1 patients, 33% of group 2, and 26% of group 3 had reactions to TDI. The severity of reactions (minimal, mild, and moderate, respectively) was as follows: group 1-6, 6, and 2; group 2-1, 5, and 0; group 3-5, 1, and 0. Data were analyzed by the two- sided Fisher's exact test using 95% confidence intervals with the approximation of Woolf. These data demonstrate that administration of MP before and after TDI reduces the frequency and severity of the arthralgia-myalgia syndrome. We conclude that 125 mg IV MP should be given routinely before and after TDI of iron dextran. Like many other acquired diseases from the environment (not organic ones) this is a possibility, not to be discounted entirely. The person with Fibro-myalgia thought it was caused by her silver fillings, so she had all replaced, and her fibro-myalgia was better. So whatever suits a person, who is to say? But I am certain of my anti-depressant theory. (She was also friendly off and on with a friend of both of us on anti-depressants. I backed off the friendship, as I try to do, (not always successfully) if I know someone is on a stimulant. I'm sure there are people who will moan about this but they do not see the rollercoaster of emotions they project on people around them. CB Not only do they produce a roller coaster of emotions, they will produce a roller coaster of damage to the health of the person they are mind connected to in the form of Crohns Disease. Very difficult to detect at times, but a particularly astute person will notice the connection. Best thing it not to form any friendship with a person on a stimulant including Xanax. Difficult in these times. Also we are not the sole person responsible for the friendship. The person on the stimulant may have thoughts of the person trying to disconnect ( I think it is not an intentional act) for some time afterwards until the friendship becomes very empty by time and circumstances. Very difficult in this time when so many are on anti-depressants, it becomes a lonely life if there are not many other friends not on anti-depressants. One must experience the misery of crohns to understand it. It is a problem that will engulf more and more persons as the problem multiplies until needless illnesses and emotions engul the innocent. And to make it worse yet, distance makes no difference. The thoughts and harm continues no matter where one goes, adding to the problems of disconnecting. Unrecognized until the damage is intense. You are smart to recognize this. <My advice is to stay away from all persons on anti-depressants or any stimulant unless it is a very casual relationship, and even then I back away and try not to form a mind connection. That of course, depends on two people (sometimes more) and so while we may back off, the other person maintains mind thoughts of us over which we have no control, except to wait until the empty friendship wears off. Difficult to understand but that is the crux of the problem of all anti-depressants and stimulants and that includes Xanax. Only tranquillizers are safe. Well frankly, Mister President, it's the **my** part. By the way, last week I was chatting with a fibromyalgia patient about her condition and her iron levels. Something she was well aware of since she had just been verified as being iron depleted, with negative results for hemosiderin in bone marrow fragments. The doctor was encouraging her to consume more meat-based protein, because her non-heme iron intake was only about half as bioavailable. We chatted about your notion of high iron being the reason for her pains. She rolled her eyes and asked me to tell you that you are an idiot and don't know what you are talking about. Sixty-five patients, 34 women and 31 men, ages 36 to 80 years, received either normal saline before and after TDI (group 1), 125 mg IV MP before and saline after TDI (group 2), or 125 mg IV MP before and after TDI (group 3). Patients were observed for 72 hours and reactions were recorded and graded according to severity. Fifty-eight percent of group 1 patients, 33% of group 2, and 26% of group 3 had reactions to TDI. The severity of reactions (minimal, mild, and moderate, respectively) was as follows: group 1-6, 6, and 2; group 2-1, 5, and 0; group 3-5, 1, and 0. Data were analyzed by the two- sided Fisher's exact test using 95% confidence intervals with the approximation of Woolf. These data demonstrate that administration of MP before and after TDI reduces the frequency and severity of the arthralgia-myalgia syndrome. We conclude that 125 mg IV MP should be given routinely before and after TDI of iron dextran. In this study, 85 female patients with primary fibromyalgia and 80 age-, height-, and weight-matched healthy women were evaluated for oxidant/antioxidant balance. Malondialdehyde is a toxic metabolite of lipid peroxidation used as a marker of free radical damage. Superoxide dismutase is an intracellular antioxidant enzyme and shows antioxidant capacity. Pain was assessed by visual analog scale. Tender points were assessed by palpation. Age, smoking, body mass index (BMI), and duration of disease were also recorded. Malondialdehyde levels were significantly higher and superoxide dismutase levels significantly lower in fibromyalgic patients than controls. Age, BMI, smoking, and duration of disease did not affect these parameters. We found no correlation between pain and number of tender points. In conclusion, oxidant/antioxidant balances were changed in fibromyalgia. Increased free radical levels may be responsible for the development of fibromyalgia. These findings may support the hypothesis of fibromyalgia as an oxidative disorder. Recent years added new information to our understanding of FM pathophysiology. Researches on genetics, biogenic amines, neurotransmitters, hypothalamic-pituitary-adrenal axis hormones, oxidative stress, and mechanisms of pain modulation, central sensitization, and autonomic functions in FM revealed various abnormalities indicating that multiple factors and mechanisms are involved in the pathogenesis of FM. Oxidative stress and nitric oxide may play an important role in FM pathophysiology, however it is still not clear whether oxidative stress abnormalities documented in FM are the cause or the effect. This should encourage further researches evaluating the potential role of oxidative stress and nitric oxide in the pathophysiology of FM and the efficacy of antioxidant treatments (omega-3 and -6 fatty acids, vitamins and others) in double blind and placebo controlled trials.
