This isn't just about my case. It's about an epidemic of hazardous
negligence in medical records. I've seen it my own case, which I assume is
typical, and I also see it partially acknowledged here:

The government is taxing people billions of dollars to fund hazardous
quacks incompetent at even the most basic clerical tasks.
It's perhaps more obvious to me than to most people, because people want
to believe that their doctors are competent at the most basic clerical
tasks, and this prejudices them. But I have tape-recorded numerous
doctors, so I can't fool myself like most patients do.
I don't talk about violence just for the sake of dramatic effect. I
literally believe that doctors at large are so complacent with their own
clerical incompetence, that there's no alternative besides violence to
encourage doctors to think twice before neglecting to keep accurate
medical records. No ones offered any alternative.
