Actually, semisitting closes the birth canal up to 30%.  For medical
references from early last century - and for the simple biomechanics,
see Gastaldo TD. Letter. Birth. 1992;19(4):230-1.

Obviously, closing the birth canal the "extra" up to 30% (semisitting)
does not make delivery safer and effective.

LITHOTOMY...

Semisitting is sort of like lithotomy.

You write regarding the lithotomy position:

"Requiring mothers to deliver babies flat on their backs (lithotomy
position)...obstructs the normal birth process..."

Semisitting closes the birth canal with more force than the lithotomy
position, because the mother is putting more weight on her sacrum.

BY USING SEMISITTING AND LITHOTOMY...

MD-obstetricians are routinely closing birth canals up to 30%.

MD-obstetricians are routinely KEEPING birth canals closed the "extra"
up to 30% (keeping women semisitting or dorsal) when babies get stuck.

MD-obstetrician experts have been LYING to cover-up.

If so, the MD-obstetrician caused cephalopelvic disproportion/CPD.

TRUE CPD IS REALLY RARE?

You write of CPD/cephalopelvic disproportion:

"CPD (cephalopelvic disproportion)...a condition where the outlet
through the pelvis is too small for the baby to pass through. When this
is truly so, a cesarean section is required. But true CPD is really
rare yet CPD is cited as the reason for 30% of all cesarean

True CPD is ROUTINE!  MD-obstetricians cause it - routinely!
MD-obstetricians truly ARE closing birth canals up to 30% - routinely -
and when babies get stuck they truly are KEEPING birth canals closed
the "extra" up to 30%.  This is IATROGENIC true CPD - complete with
MD-obstetrician cover-up lies which perpetuate it.

SQUATTING...

I was so pleased to read your statement that,

pelvis to open wider than normal to allow the baby through."

SIDE-LYING...

How cool that in a subsequent birth, "Lori was...instructed to roll
onto her left side for pushing...part of the reason for pushing in this
position was to...avoid further elevating her blood pressure..."

Why not mention that - like squatting - side-lying allows the birth
canal to open the "extra" up to 30%?

Why not mention that - like squatting - almost EVERY delivery position
- except lithotomy and semisitting - allow the birth canal to open the
"extra" up to 30%?

In The Cultural Warping of Childbirth, Doris Haire called semisitting
"the physiologic position for childbirth."

Back in the 90s when I informed Doris of her gross error, she agreed it
was gross error but never spoke out about it.

Same thing happened with Jay Hathaway of Bradley Method/American
Academy of Husband Coached Childbirth/AAHCC who may STILL be promoting
semisitting as "the Bradley Classic" in the Bradley Method Student
Workbook.

I did not mention that homebirth midwives are also promoting
semisitting/birth-canal-closing...

Midwifery Today editor Jan Tritten called "semisitting" a "Midwifery
Trick of the Trade" after I informed her of the simple biomechanics.
And in cases of shoulder dystocia, prominent homebirth midwife Ina May
Gaskin recommends semisitting (McRoberts) before trying hands-and-knees
("the Gaskin maneuver").

David, I am glad that you may possibly revise your article.  I will
post your reply (and this response) to the usenet.

PS  It is an obvious crime of negligence for MD-obstetricians to ignore
their own literature and simple biomechanics.  The crime occasionally
escalates to criminally negligent homicide and is only made worse by
MD-obstetrician experts lying to cover-up.

I am in favor of pardons in advance for MDs.  As medical students, MDs
are TRAINED to perform obvious felonies (plural).

Thank you for your email regarding the semi-sitting position. The
reference to "semi-sitting" comes from the source I was quoting at that
point : The Cultural Warping of Childbirth, A Special Report on U.S.
Obstetrics prepared for the International Childbirth Education
Association by Doris B. Haire, D.M.S. (as noted in the article).

I supposed semi-sitting was meaning a position close to that of
squatting and thus similar in effectiveness. If I am understanding your
comments correctly, this is not at all the case.

I will plan to take a little time to look into this matter further, and
then possibly revise my article accordingly.

(on her butt)?"  Before her 4 c-sections she labored on her back.

As far as CPD - I was referring to the pelvis being too small for the
baby to pass through in ANY position. This is how I have seen this
condition referred to - not as a problem caused by a poor position for
labor. So this is why I say "true CPD is really rare".  My point is
that most women who are given this excuse are being led to believe that
there is no way they can deliver that size of baby, whereas they could
under differing laboring conditions. I agree with you that
obstetricians may be causing such a problem with the way they manage
the woman's labor.

##### DR. GASTALDO REMARKS:  Yes, when MD-obstetricians say they did a
c-section because of "CPD" (you say they do this in 30% of c-sections)
we should take them at their word and ask why they are causing CPD -
closing birth canals up to 30% not to mention KEEPING birth canals
closed the "extra" up to 30% when babies get stuck.  Actually, people
shouldn't wait until an MD-obstetrician is doing a c-section before
asking this important question - esp. since the answer is obvious:
MD-obstetricians can't simply stop the crime - because stopping it
would be tantamount to admitting it.
