The Ultimate Oxymoron: Immune Reconstitution Syndrome I love this one - from the CDC - here the PharmaSpin takes a leap off the cliff of reality as it describes how it is a GOOD thing when OIs get WORSE when the hapless patient begins taking antiviral drugs! Of course, hepatitis replicates like CRAZY when ARV commences and many people have had their livers destroyed by these wonderful "life-saving" drugs... sorry the patient died, but at least his immune system was reconstituted! Gotta love how you take these drugs to get better, but it's now OK to get WORSE! susie ----- http://www.cdc.gov/mmwr/preview/mmwrhtml/rr5315a1.htm Immune reconstitution syndromes have been described for mycobacterial infections (including disease caused by Mycobacterium avium complex [MAC] and Mycobacterium tuberculosis, Pneumocystis jiroveci pneumonia (PCP), toxoplasmosis, hepatitis B and hepatitis C viruses, cytomegalovirus (CMV) infection, varicella-zoster virus (VZV) infection, cryptococcal infection and PML (12,15--25). Immune reconstitution syndromes are characterized by fever and worsening of the clinical manifestations of the OI or new manifestations weeks after the initiation of ART. Determining the absence of recrudescence of the underlying OI and new drug toxicity or a new OI is important. If the syndrome does represent an immune reactivation syndrome, adding nonsteroidal anti-inflammatory agents or corticosteroids to alleviate the inflammatory reaction is appropriate. The inflammation might take weeks or months to subside.
